In vitro evaluation of sodium thiosulfate, at different concentrations and times of application, in the bond strength to intracoronary dentin treated with sodium hypochlorite

A resistência de união de sistemas adesivos à dentina é comprometida quando esta é tratada com hipoclorito de sódio (NaOCl). Soluções antioxidantes têm sido propostas a fim de restabelecer os valores de adesão. Embora vários estudos tenham avaliado a eficiência do ascorbato de sódio, outras substâncias devem ser pesquisadas. O objetivo deste estudo foi avaliar a eficácia do tiossulfato de sódio (Na2S2O3), em diferentes tempos e concentrações, no restabelecimento da resistência de união à dentina tratada com NaOCl. Coroas de dentes bovinos foram cortadas de forma a expor a dentina da câmara pulpar. As superfícies dentinárias foram tratadas como se segue: grupo 1- cloreto de sódio a 0,9% durante 30 min.; grupo 2: NaOCl a 5,25% durante 30 min., EDTA a 17% durante 3 minutos e mais NaOCl a 5,25% durante 1 min.; nos grupos 3, 4, 5, 6, 7 e 8 foi realizado o mesmo protocolo do grupo 2, porém seguido da aplicação de Na2S2O3 a 0,5% por 1 min. (grupo 3), 5 min. (grupo 4) e 10 min. (grupo 5) e de Na2S2O3 a 5% por 1 min. (grupo 6), 5 min. (grupo 7) e 10 min. (grupo 8); grupo 9- imersão em cloreto de sódio a 0,9% por 10 min., após o mesmo tratamento realizado no grupo 2. Depois disso, o sistema adesivo Scotchbond Multipurpose foi aplicado na dentina da câmara pulpar, seguido da resina composta Z250. Após armazenamento em 100% de umidade por 24 horas a 37 ºC, 25 palitos foram obtidos a partir das amostras de cada grupo e submetidos ao ensaio de microtração. Os dados convertidos em MPa foram analisados por ANOVA e teste de Fisher (p <0,05). Os grupos 2 e 9 apresentaram resistência de união significativamente mais baixa do que a do controle (grupo 1). Não houve diferença estatisticamente significativa entre a aplicação da solução de Na2S2O3 a 0,5% ou 5%, durante 5 ou 10 min., para aumentar a resistência de união à dentina comprometida pelo tratamento com NaOCl (p> 0,05). Não houve diferença estatisticamente significativa entre os grupos 1 e 8 (p = 0,944), mas ambos são estatisticamente diferentes dos grupos 2, 3 e 9. Todos os grupos estudados apresentaram dois ou mais padrões de fratura, sendo que o predominante foi o coesivo em camada híbrida. Observou-se que os grupos que apresentaram maior resistência de união (Grupos 1 e 8) foram os que apresentaram o maior percentual de fraturas do tipo coesiva em resina ou dentina. Pode-se concluir que o NaOCl reduziu significativamente a resistência de união à dentina da câmara pulpar. O restabelecimento da resistência de união à dentina comprometida pelo tratamento com NaOCl não dependeu da concentração do Na2S2O3, quando este foi usado por 5 ou 10 min. Depois de aplicar por 1min, o Na2S2O3 a 5% é capaz de restaurar a resistência de união à dentina tratada com NaOCl.The bond strength to dentin is compromised when it is treated with sodium hypochlorite. Several antioxidant solutions have been proposed in order to reestablish the adhesion values. Although several studies have evaluated the efficiency of sodium ascorbate, other substances should be assessed. The purpose of this study was to evaluate the antioxidant efficiency of sodium thiosulfate (Na2S2O3), at different times and concentrations, in reestablishing the adhesive bond strength to sodium hypochlorite-treated dentin. Crowns of bovine incisors were cut to expose the pulp chamber. The dentin surfaces were treated as follows: group 1: 0.9% sodium chloride for 30 min; group 2: treated with 5.25% sodium hypochlorite (NaOCl) for 30 min, 17% EDTA for 3 min and additional 5.25% NaOCl for 1 min; groups 3, 4, 5, 6, 7 and 8: followed the same treatment performed in group 2, adding the final applications of 0.5% sodium thiosulfate solution for 1 min (group 3), 5 min (group 4) and 10 min (group 5) or applications of 5% sodium thiosulfate solution for 1 (group 6), 5 (group 7) and 10 min (group 8); group 9: immersion in 0.9% sodium chloride for 10 min after the same treatment performed in group 2. After that, Scotchbond Multipurpose adhesive system was applied to pulp chamber dentin, followed by Filtek Z250 composite. After storage in water for 24h at 37oC, twenty-five rectangular sticks were obtained from the specimens of each group and subjected to the microtensile bond test. The data converted to MPa were analyzed by one-way ANOVA and Fisher's test (p<0.05). The groups 2 and 9 had significantly lower bond strength than the control (group 1). There was no statistically difference between application of 0.5% or 5% sodium thiosulfate solution for 5 or 10 min to increase the compromised bonding to NaOCl-treated dentin (p>0.05). There was no statistically significant difference between groups 1 and 8 (p = 0.944), but both are statistically different from the groups 2, 3 and 9. All groups studied presented two or more failure mode and the fractures predominating are the cohesive in the hybrid layer. It was observed that the groups that showed the highest bond strength (Groups 1 and 8) were the ones with the highest percentage of cohesive resin / dentin fractures type. It was concluded that sodium hypochlorite significantly reduced the bond strength to pulp chamber dentin. The bond strength reestablishment on compromised NaOCl-treated dentin do not depended on the sodium thiosulfate concentration when it is used for 5 and 10 min. After applied for 1min, 5% sodium thiosulfate is able to restore the bond strengths to NaOCl-treated dentin
