(A) study on the strategy of domestic hospital industry preparing for the opening of medical service

보건관리학과/석사[한글] 우루과이라운드협상 발효 및 외국인 투자개방계획의 의료서비스 개방에 따른 국내 병원산업에 대한 영향을 분석하고, 정부의 의료정책 수행에 따른 제도적 측면 및 병원차원의 경쟁력 열세에 따른 현황과 과제를 파악하여 국내 병원산업의 개방 여파를 최소화 할 수 있는 효율적인 대응전략을 제시하기 위하여 기존의 우루과이라운드 서비스협상에 대한 연구와 각종 의료서비스 개방에 관련된 연구자료를 분석한 주요한 결과는 다음과 같다. 첫째, 의료서비스 개방이 국내 병원산업에 미치는 영향은 의료체계, 병원, 소비자의 측면으로 구분할 수 있다. 의료체계에 미치는 영향으로서도, 농간 의료자원의 불균형 심화, 왜곡된 진료패턴으로 인한 기존 의료전달체계의 혼란을 들 수 있다. 병원에 미치는 영향 으로는 선진국의 병원관리기법의 이전을 통한 병원경영합리화 추구, 의료인력의 과열 스카웃으로 인한 인력수급문제 유발, 중소병원의 경영악화, 외국 의료기관의 의료분쟁 및 소송사례의 관대한 보상의 영향으로 인한 병원경영 압박을 들 수 있다. 소비자에게는 의료선택의 폭을 넓힘으로서 다양한 의료서비스에 대한 욕구를 충족시켜 줄 수 있으나 잠재의료수요 창출로써 국민의료비의 양등에 직접적인 영향을 미칠 것이다. 둘째, 국내 병원산업의 현황과 과제로는 의료전달체계의 왜곡, 지역별 의료자원의 불균형, 의료수가 및 진료비 심사제도의 불합리, 의료환경이 공급자중심에서 소비자중심으로 변화하는 환경에 대한 의료인의 사고전환미흡, 생산성 및 수익성 측면에서의 비효율적인 병원관리, 중소병원의 열악한 경쟁력 등을 들 수가 있다. 셋째, 의료서비스 개방에 대한 국내 병원산업의 대응전략은 정부차원의 제도적 장치의 마련과 병원차원의 경쟁력 제고방안으로 구분할 수 있다. 정부차원의 제도적 장치의 마련은 의료 및 관계법령의 보완과 개방대응적인 정책수행으로 구분할 수 있으며, 병원차원의 경쟁력제고 방안은 의료서비스의 질 향상과 병원관리의 효율성 증대로 구분할 수 있다. 넷째, 정부차원의 제도적 장치의 마련은 의료 및 관계법령의 보완을 통한 의사면허 및 병원시설의 투자에 국제규범상 수용할 수 있는 규제 방안을 강구하는 것이며, 개방대응적인 의료정책수행은 의료전달체계의 정립, 효율적인 지역별 의료자원의 수급계획 마련, 의 료수가 및 진료비 심사제도의 개선 등을 들 수가 있다. 다섯째, 병원차원의 경쟁력제고 방안은 의료서비스의 질 향상을 위하여 의료인의 소비자 중심으로의 인식전환, 의료의 총체적 질 관리, 중소병원의 전문화, 중소병원과 대형병원의 모자관계수립 등을 들 수가 있으며 병원관리의 효율성 중대를 위하여는 전문경영인에 의한 책임경영제 도입, 병원합동관리제도에 의한 원가절감, 병원경영의 다각화 등을 들 수가 있다. [영문] Through the literature review, this study analyzed that the impacts of the opening of medical service sector on hospital industry and have a through grasp of the present status and task of Koran hospital industry for the purpose of establishing coutermeasures which improve efficient hospital administration against the opening of medical service. By analyzing data of UR service negotiation and studies which being related to opening of medical service, the following results were obtained: 1. The impacts of the opening of medical service on domestic hospital industry can be seen in the aspects of medical system, hospitals and consumers, respectively. 2. The problems of the opening of medical service sector on Korean hospital industry is distortion of health care delivery system, disproportion of regional medical resources, absurdity of medical charge and a doctor's bill judgement system, the insufficiency of diverting recognition of changing external situation, inefficient hospital management and poor competition of small and medium hospital. 3. Countermeasures of the opening of medical service sector on domestic hospital industry is classified preperation of governmental systematic mechanism and a elevation of hospital administrative competition. 4. Preperation of governmental systematic mechanism is preperation of restrictive regulation for doctor's license and investment of medical facilities within UR regulation. Framing a measured medical policy is achieved by taking a establishment of health care delivery system, making a supply and demand program for efficient local medical resources and improving medical charge and a doctor's bill judgement system. 5. Elevation of hospital administrative competition is made by changing recognition to consumer, total quality management of medical service, specialization of small and medium hospital, relationship between small and medium hospital with large hospital. Raise of efficient hospital management is made by taking responsibility administration system by professional hospital administrator, multihospital system, and diversification of hospital administration.restrictio
