The plateau of incident rates in many domains, including road and rail transport, aviation and workplaces indicates that system-based ergonomics models used to understand accident causation are either underutilised or underperforming in preventing adverse events. Whether this plateau is due to an increase in system complexity or out-of-date toolkits, the problems are seemingly resistant to the current ergonomics methods that pursue them. With this knowledge it is pertinent to ask what changes, if any, are required to capture contemporary system complexity and uncover resistant problems.
