Prevention of Wisdom Medical Benefits of Falling Risks for Stroke Patients-A Case Study of Rehabilitation Department of a Regional Teaching Hospitals in Central Taiwan

[[abstract]]The purpose of this study is to explore the effectiveness of the prevent-falling wisdom medical module in patients with stroke by reducing the risk of home falling. According to the results of the literature, we have developed a Prevent-falling- wisdom- medical- module(PFWMM): 1. Peer group of stroke patients – Prevent- falling education, and home environment improvement education. 2. After the medical records of electronic, through the Tableau software will visualize to the graphic indicators - to facilitate the communication and suggestion of home environmental transformation. 3. Home environment visit with a combination of wearing intelligent devices and applications app - to achieve a more accurate assessment and transformation of the safe home environment. The author conducted a cross-sectional study of 183 stroke patients discharged from a regional teaching hospital in central Taiwan to investigate whether there was any fall within one year, whether there was any injury, whether there was a home improvement. The results found that the home falling rate of the experimental group A is 13.79% and the injury rate is 10.34% of the 87 patients who participated in the Post-acute- care program (PAC) with the prevent-falling- wisdom- medical- module(PFWMM), were not only better than the literature of home falling rate of 37% and the injury rate of 22 %, also better than the 39 patients of the control group A (participated in the PAC program without the PFWMM) with 23.08% falling rate and 15.38% injury rate, and better than the 57 patients of the control group B( not participated in the PAC program) with 29.82% falling rate and the injury rate of 24.56% (p <0.05). For PAC patients with high risk of falling (the experimental group A-1), who have been assessed by a Berg Balance Scale (BBS) < 40 or walking speed< 0.8 M / s. We will inform patients and caregivers with the graphic indicators and suggest having home environmental transformation, and 10.34% falling rate as a result reduced to the level of general PAC patients (13.79%). The home environmental transformation rate fell by 3.33%, lower than the 17.86% who did not have the home environment transformation. So, the PFWMM can effectively reduce the risk of home falling risk of the stroke patients
