I saw my first patient with severe immune deficiency in 1979 – a very low CD4 count had been noted, but it was not until the first reports of an epidemic occurred in 1981 that the correct diagnosis was made. Subsequently, I have seen more than 15,000 patients with HIV-related immune deficiency, and my life has changed from helping terminally ill patients to die with dignity, in the early part of the epidemic to now providing drugs for an eminently treatable condition – a true miracle. I have a number of observations about the epidemic.
