Évolution de l’organisation du système de santé. Inefficacité, violence et santé numérique

International audienceChina’s public health system has gone through a number of development stages. This paper aims at showing how, from its inception as a hierarchical system, the healthcare system then lost its structure, to finally give birth to a focalised system where the first point of entry in the treatment process has become the hospital, in particular the healthcare establishments that offer the most reliable standards of care. These days, the widely-acknowledged inefficiencies of the healthcare system have led to a climate of violence between medical staff and their patients, caused by the overwhelming demand that hospitals must bear, the ambiguous status of public establishments, financial benefits and other perks for medical staff, as well as the medical staff’s civil servant status, and the implications thereof. Policies to foster the development of primary health centres are struggling to gain traction, while digital healthcare offers promising solutions and is developing fastL’organisation du système de santé chinois a connu plusieurs phases de développement. L’objectif de cet article est de montrer comment d’un système hiérarchisé, le système de santé s’est déstructuré puis a donné naissance à un système centralisé où le point d’entrée du parcours des soins est devenu l’hôpital, en particulier les centres hospitaliers offrant un niveau de qualité des plus performants. Le système de santé, se caractérisant alors par son inefficacité, a conduit à une forte violence entre le corps médical et la population. Les raisons en sont l’engorgement de l'offre hospitalière, l’ambiguïté du statut des établissements publics, les compensations financières et autres à-côtés dont bénéficie le corps médical, et le statut de fonctionnaire des praticiens, et ses implications. La mise en place de centres de santé primaires se fait avec peine tandis que la santé numérique est un marché en expansion
