Drainage efficacy after laparoscopic cholecystectomy for acute calculous cholecystitis

Aim of thethesis: To assess the benefits and drawbacks of the peritoneal cavity drainage after emergency laparoscopic cholecystectomy. Methods: A prospective analysis of emergency laparoscopic cholecystectomy. The participants-patients operated on for acute cholecystitis (ICD-10-AM: K80.00). The study included 20 patients allocated into two groups; group A (n=10) patients with intraoperative drain insertion and group B (n=10) patients without drain insertion. Patients signed informed consent form for this study. Clinical examination was performed, data from hospital case files were analyzed and abdominal ultrasonography was performed on the first postoperative day with the goal to detect any fluid collection. Pain intensity was assessed using the Visual Analogue Scale (VAS). Duration of drainage, amount and type of secretion through drain were evaluated and the relation between postoperative complications and peritoneal cavity drainage was assessed. Duration of surgery, length of hospital stay, cost of treatment, as well as postoperative period ultrasonographic findings were compared. Results: age, body temperature and leucocyte count were not statistically different, however, in group A a significantly higher CRP concentration was found. Duration of surgery did not differ between the groups. There was a longer length of hospital stay in group A. Neither group experienced postoperative complications. The relationship between CRP concentration and duration of surgery was found. The intensity of pain was not different, however, after drain removal the pain decreased significantly. A more frequent intraabdominal free fluid collection was found in group A. The median duration of drainage was 2 days and the mean secretion volume was 65 ml
