Analysis of treatment results for penetrating abdominal trauma

Aim – to evaluate the penetrating abdominal trauma features, severity and treatment out-comes, based on current standards of care. Objectives: to assess how organs injuries and injuries severity (by ISS scale) affect treatment complications; to evaluate postoperative complications and hospital bed-days in relation to injuries se-verity (by ISS scale); to find the possible prognostic factor, suggesting about penetrating abdominal trauma with organs injuries. Methods: This was a retrospective study. We analysed case histories of patients who diag-nosed and treated due to penetrating abdominal trauma in the Department of Surgery, Hospital of Li-thuanian University of Health Sciences Kaunas Clinics from 2006 to 2016. We took data about pa-tients’ age, gender, trauma kind, vital signs, laboratory and radiology tests, time from injury to opera-tion, duration of treatment, treatment outcomes and postoperative complications. Results: The study included 93 patients: 81 male (87,1%) and 12 female (12,9%). The mean age was 38,9 (20) yr. Mean age of men – 38,1 (15,2) yr. and women – 45,4 (18) yr. All patients had penetrating abdominal trauma. We obtained that the main part of patients had penetrating abdominal trauma with organs injuries and only 10% of patients did not had organs injuries. After assessment according ISS scale, the most difficult patients were with penetrating abdominal trauma with organs injuries. That kind of patients spent more time in hospital (2,5 time more) and more commonly had postoperative complications. Conclusions: 1. Complications were rare. When ISS score was more than 15 and patients had penetrating abdominal trauma with organs injuries, we obtained the greater amount of complications. 2. When ISS >15 the postoperative complications were most common and spent more time in the hos-pital. 3. When SBP less than 101 mmHg, leukocytes more than 9,1 x 109/l, hemoglobin less than 110 g/l, C-reactive protein more than 10 mg/l and during abdominal ultrasound or CT scan detected changes is 3,4 times more likely to have penetrating abdominal trauma with organs injuries
