Survival of direct composites restorations, veneers and ceramic veneers in anterior teeth: $b retrospective study of factors related to the replacement and repair

The evaluation of restorations survival in posterior teeth has been quite explored in the recent literature, however, few studies were found on the longevity of restorations in anterior teeth. This study evaluated the longevity of direct composite restorations, direct veneers and ceramic veneers in anterior teeth, investigating factors related to failure and repair of these restorations. The study design was a retrospective longitudinal, conducted by collecting data from the files of a private office. For inclusion, patients should have received restorations in anterior teeth, composite resin or ceramic veneers, and have remained in the same monitoring dental practice, the period included in the review was from 1994 to 2014. The restorations and evaluations were performed by a single operator. The data evaluation was divided into two stages. In the first part of the study, composites with 5-20 years of follow-up were selected. The factors were sex of the patient, the type of composite resin (hybrid, microhybrid, and microfilled and Hybrid association), the arch (upper and lower), the tooth (central incisor, lateral incisor and canine), and type cavity (Class III and IV and facet). The outcome measures were the interventions (repair or replacement of restorations) and annual failure rate (considering repair or replacement as failure). Statistical analyzes were performed using chi-square tests, survival analysis using the Kaplan-Meier method and Cox regression. The second part of the study included the veneer restorations with follow-up times of up to four years, held in composite nanohybrid / nanofilled and ceramics, from 2009 until 2013. Only descriptive statistics could be used. In total, 123 patients were enrolled, and 524 restorations were evaluated in the first part. Significant differences for replacement of restorations were found between dental arches, with upper arch with higher failure rate compared to lower, after 10 years of follow-up, showed a survival rate of 41% and 73% survival, respectively. About repairs, significant differences were found according to the material used, the tooth and the type of restoration, with microhybrids resins possessing a larger amount of repairs in the relation of hybrid and more hybrid microparticulate association, however with greater survival in relation to the other. Greater frequency of repair were found between the types of restoration was carried out, with veneers having greater amount of repairs in relation 9 to restorations class III and IV. In the second part of the study, compared direct and indirect veneers, 155 facets were evaluated, only descriptive statistics were used, there were no differences between the techniques regarding the replacement and repair of the facets with up to four years of follow-up, despite the ceramics evaluated have a higher amount of replacement and repairs. Even with the limitations of this study, that possible conclude that aesthetics can be the main cause for failure of restorations in anterior teeth, restorations on the upper teeth had a greater number of interventions in relation to the lower. In relation to comparison of veneers, the direct and indirect techniques had similar and satisfactory results with four year follow-up, however a greater number of facets and longer follow-up is necessary to highlight the major differences.Coordenação de Aperfeiçoamento de Pessoal de Nível Superior - CAPESA avaliação da sobrevivência de restaurações em dentes posteriores tem sido bastante explorada na literatura recente, no entanto, poucos estudos foram encontrados sobre a longevidade de restaurações em dentes anteriores. O presente estudo avaliou a longevidade de restaurações e facetas de resina composta direta e facetas cerâmicas em dentes anteriores, investigando alguns fatores relacionados à falha e ao reparo destas restaurações. O desenho do estudo foi do tipo longitudinal retrospectivo, realizado por meio da coleta de dados dos arquivos de um consultório privado. Para inclusão, os pacientes deveriam ter recebido restaurações em dentes anteriores, em resina composta ou facetas cerâmicas, e ter permanecido em acompanhamento na mesma clínica odontológica, o período incluído na avaliação foi de 1994 até 2014. As restaurações e avaliações foram realizadas por um único operador. A avaliação dos dados coletados foi dividida em duas etapas. Na primeira parte do estudo, resinas compostas com 5 a 20 anos de acompanhamento foram selecionadas. Os fatores em estudo foram o sexo do paciente, o tipo de resina composta (Híbrida, Microhíbrida, e associação Microparticulada e Híbrida), o arco (superior e inferior), o dente (incisivo central, incisivo lateral e canino), e o tipo de cavidade (classe III e IV e faceta). Os desfechos avaliados foram às intervenções realizadas (reparo ou substituição das restaurações) e taxa de falha anual (considerando reparo ou substituição como falha). As análises estatísticas foram realizadas com testes Qui-quadrado, análise de sobrevivência pelo método Kaplan- Meier e Regressão de Cox. A segunda parte do estudo incluiu as restaurações do tipo faceta com tempos de acompanhamento de até 4 anos, realizadas em resina composta nanohíbrida/nanoparticulada e cerâmicas, no período de 2009 até 2013. Apenas estatística descritiva pôde ser utilizada. No total, 123 pacientes foram incluídos, e 524 restaurações foram avaliadas na primeira parte. Diferenças significativas para substituição das restaurações foram encontradas entre os arcos dentários, com arco superior tendo maior taxa de falha em relação ao inferior, após 10 anos de acompanhamento, apresentavam uma taxa de sobrevivência de 41% e 73% de sobrevivência, respectivamente. Sobre os reparos, diferenças significativas foram encontradas de acordo com o material utilizado e o tipo de restauração, com as resinas microhíbridas apresentando maior quantidade de reparos em relação às híbridas e a associação micropartículadas mais híbridas, entretanto com maior sobrevivência em relação as outras. Foram encontradas maiores frequências de reparo, entre os tipos de restauração realizada, com facetas apresentando maior quantidade de reparos em relação às restaurações classe III e IV. Na segunda parte do estudo, na comparação de facetas diretas e indiretas, 155 facetas foram avaliadas, apenas estatística descritiva foi utilizada, não foram encontradas diferenças entre as técnicas com relação a substituição e reparo das facetas com 7 até quatro anos de acompanhamento, apesar das cerâmicas apresentarem maior quantidade de substituição e reparos. Mesmo com as limitações do presente estudo, se conclui que estética pode ser a principal causa para falhas das restaurações em dentes anteriores, restaurações nos dentes superiores apresentaram maior número de intervenções em relação aos inferiores. Já com relação a comparação entre facetas, as técnicas direta e indireta apresentaram resultados semelhantes e satisfatórios com quatro anos de acompanhamento, entretanto um maior número de facetas e maior tempo de acompanhamento é necessário para evidenciar maiores diferenças
