Assessment of the relationship between kisspeptin levels and insulin resistance, oxidative stress parameters and ovarian androgenism in polycystic ovary syndrome

Ancak, PKOS etyolojisi tam olarak aydınlatılamamıştır. Bu sendromun, primer olarak over kaynaklı bir bozukluk olduğu kabul edilmiş ve bu hastalık tablosunu oluşturan nedenler konusunda, birçok mekanizma ve teori öne sürülmüştür. Kisspeptin, 54 amino asitten oluşan bir proteindir. Kisspeptin ve hipotalamo hipofizer gonadal aks arasındaki ilişki PKOS‟lu hastalardaki GnRH sekresyonundaki değişikliklerle ilişkili olabileceği düşünülmektedir. Bu çalışmada yeni tanı almış polikistik over sendromlu 37 hasta ile sağlıklı kontrol grubu olarak değerlendirilen 38 olgu analiz edilmiştir. Hasta ve kontrol gruplarında kisspeptin, glutatyon peroksidaz ve süperoksit dismutaz seviyeleri arasında anlamlı farklılık saptanmamıştır. Ancak ortalama katalaz seviyesi PKOS grubunda, kontrol grubuna kıyasla anlamlı şekilde düşük saptanmıştır (285,73 ± 26,10 vs. 382,30 ± 39,76, p<0,05). PKOS grubu analiz edildiğinde, kisspeptin ile homa, serbest testosteron ve DHEA-S seviyeleri arasında anlamlı korelasyon izlenmemiştir. Ancak katalaz seviyelerinin kisspeptin ile istatistiksel olarak anlamlı bir şekilde korele olduğu gözlenmiştir (r=0,340, p<0,05). Kisspeptinin patofizyolojideki gerçek rolünü anlamak ve metabolik ilişkilerini çözümleyebilmek için PKOS fenotipleri ve alt gruplarını daha geniş serili bir şekilde değerlendirecek yeni çalışmalara ihtiyaç duyulmaktadır.Polycystic ovarian syndrome (PCOS) is the most common endocrine disorder in women in the reproductive period. However, the etiology of PCOS has not been fully elucidated. Although, the disease is mainly associated with ovarian pathology, several other mechanisms and theories have been proposed for the underlying mechanisms. Kisspeptin is a protein consisting of 54 amino acids. The association between the kisspeptin and the hypothalamo hypophyseal gonadal axis is thought to be related to changes in GnRH secretion in patients with PCOS. In this study, 37 patients with newly diagnosed Polikistic Ovary Sydrome and 38 healthy control groups were analyzed. There was no significant difference between the levels of kisspeptin, glutathione peroxidase and superoxide dismutase in patients and the control group. However, the average catalase level was significantly lower in the PCOS group compared to that of the control group (285,73 ± 26,10 vs. 382,30 ± 39,76, p <0,05). When the cases of the PCOS group was further analyzed, no significant correlation between kisspeptin and HOMA, free testosterone and DHEAS levels were observed.
