Functional characterization of affinity purified anti-ADAMTS13 IgG autoantibodies from patients with acquired thrombotic thrombocytopenic purpura and a healthy donor pool

Gegen ADAMTS13 gerichtete neutralisierende Immunglobulin G (IgG) Autoantikörper sind die Hauptursache für die erworbene idiopathische thrombotische thrombocytopenische Purpura (aTTP). Die hier vorgelegte Doktorarbeit beschreibt die Charakterisierung von affinitätsgereinigten anti-ADAMTS13 IgGs aus Plasma von Patienten mit aTTP sowie einem Plasmapool von gesunden Spendern. Alle Präparationen enthielten die IgG Unterklassen IgG1 bis IgG4, wobei mengenmäßig IgG2 am häufigsten war, gefolgt von IgG1 und IgG3/4. Für die aus aTTP Patienten gereinigten anti-ADAMTS13 IgGs wurden Dissoziationskonstanten von 1 nM bestimmt. Wie erwartet, blockierten die TTP Antikörper konzentrationsabhängig die Interaktion von ADAMTS13 mit VWF. Ähnlich gereinigte anti-ADAMTS13 IgGs aus einem gesunden Spenderpool zeigten eine schwache Bindung an ADAMTS13, inhibierten aber nicht dessen Aktivität. Eine auf dem statischen FRETS-VWF73 Aktivitätstest beruhende Inhibitortestung wies den TTP Antikörpern eine neutralisierende spezifische Aktivität von 0.1-0.5 BU/g zu. In einem neu etablierten fluiddynamischen ADAMTS13 Aktivitätstest zeigten dieselben TTP Antikörper jedoch überraschenderweise eine abweichende Beeinträchtigung der Aktivität. Anti-ADAMTS13 Antikörper könnten daher in vivo Eigenschaften aufweisen, welche mit den in statischen Tests gemessenen Inhibitor-Konzentrationen nicht übereinstimmen. Studien zur Epitopkartierung bestätigten die polyklonale Natur der isolierten anti-ADAMTS13 Antikörper und ermöglichten Einsicht in deren lineare Epitopprofile. Interessanterweise hatten die aus dem gesunden Spenderpool gereinigten IgGs einige lineare Epitope mit jenen aus TTP Patienten gemein. Dies lässt vermuten, dass die in gesunden Personen detektierten, nicht inhibierenden anti-ADAMTS13 Antikörper die Vorstufe von hochaffinen und pathogenen IgG Antikörpern in erworbener TTP darstellen. Die Arbeit liefert neue Erkenntnisse über die Entwicklung inhibitorischer Antikörper in erworbener TTP.ADAMTS13-neutralizing IgG autoantibodies (Abs) are the major cause of acquired idiopathic thrombotic thrombocytopenic purpura (aTTP). My thesis presents the first characterization of affinity-purified anti-ADAMTS13 IgGs from plasma of patients with aTTP and a plasma pool of healthy donors (HD). All preparations contained IgG subclasses IgG1 to IgG4 with IgG2 as the most prevalent, then IgG1 and IgG3/4. For the IgGs from aTTP patients, binding to recombinant and plasma derived ADAMTS13, which was purified from a cold ethanol fractionated human plasma intermediate, revealed dissociation constants of 1 nM resulting primarily from IgG1, IgG3, and IgG4. Routine static activity assays showed the TTP Abs to have a neutralizing activity of 0.1-0.5 BU/g and to interfere with the ADAMTS13-VWF interaction. In a newly established flow assay, ADAMTS13 activity was inhibited to a varying extent by two TTP antibody preparations when pre-adjusted to comparable FRETS-VWF73 based inhibitor titers. Anti-ADAMTS13 Abs may thus exhibit inhibitory properties in vivo that are not consistent with the ADAMTS13 inhibitor levels determined in routine static assays. Therefore, in possible future treatment of acquired TTP with recombinant ADAMTS13, dosing should not be based on inhibitor titers obtained by routine assays alone. Similarly purified anti-ADAMTS13 IgGs from a HD plasma pool bound ADAMTS13 weakly and did not inhibit ADAMTS13 activity. Antibody epitope mapping studies revealed the IgGs to be polyclonal and elucidated the linear epitope profile of ADAMTS13 IgG Abs. Despite being non-inhibitory, IgGs purified from the HD plasma pool shared linear epitopes with the Abs from patients with TTP suggesting that anti-ADAMTS13 IgG Abs occurring in HD may provide the template for emergence of high affinity and pathogenic IgG Abs in aTTP.
