Implant supported cantilevered dentures, advantages, disadvantages, biological and technical complications

There are many questions of using cantilevered implant –supported dentures. Cantilevers fixed on natural teeth abutments had many negative aspects, but when we started to use implant-supported cantilevers they had shown much better results. Some authors says that implant overloading is impossible. The aim of this systematic literature review: to analyse cantilever dentures advantages, disadvantages, using capabilities, influence of forces that affects dentures and implants ant technical and biological complications. A literature search was conducted in electronic PubMed, ScienceDirect databases for articles published between 2007 and 2017. Used key words: cantilever, dental implant, bone loss, complications. The literature discussing the main cantilever restorations advantages: lower cost, avoids unwanted anatomical structures, in some cases avoids surgical interventions and increases the amount of occlusive elements. The biggest drawback - it possible biomechanical complications. Based on all the studies that have been studied bone loss around the implant for the use of cantilevered prosthesis, the differences between the cantilevers and restorations without cantilevers were not statistically significant. Most of the biomechanical complications depends on the mechanical overload that caused by improper design of the prosthesis. The most common complications: marginal bone loss and loosened prosthesis retaining screw, which can lead to further complications in the chain. It also discussed the recommendations for prosthetic structures, which would reduce the likelihood of biomechanical complications, but each clinical case is unique and treatment plan should be carefully thought out and planned. It can be concluded that the cantilevered restoration does not affect the bone surrounding the implant, but must be appropriate design.
