The Longevity of Population Health

Susan L. Freeman, MD, MS, FACPE, FACE is the founding CEO and President of Temple Center for Population Health, LLC. The Center was developed to attain a sustainable model of health care delivery in North Philadelphia through clinical and business integration, community engagement, and academic distinction to promote healthy populations. She is also the Chief Medical Officer, Temple University Health System; Vice Dean of Health Care Systems and Professor of Clinical Medicine at the Lewis Katz School of Medicine at Temple University. Dr. Freeman serves on several state and local advisory boards to promote health care reform and has spoken widely on population health; high value care at safety net hospitals; and accountable care models. She is a graduate of Michigan State University College of Human Medicine and completed residency and fellowship training in endocrinology and metabolism from Mayo Graduate School of Medicine. Dr. Freeman’s Forum presentation includes an overview of the framework for addressing population health in a shifting political environment. Dr. Freeman identifies the drivers of population health and the vital nature of clinical and business integration in health care and recommends a multidisciplinary approach to create a balance of medical and nonmedical interventions to promote high value care and health populations.
