Affections psychosomatiques du cerveau : histoire d'un concept heuristique

International audiencePsychosomatic disorders are classically related to the body outside the brain. Yet, there is no reason why the circuits of the brain cannot be affected by a psychosomatic process. This instance was implied in ancient theories regarding the effect of passions on the brain, by mean of humors. They were also implied in the theories of French alienists of the beginning of the 19th century (Georget in particular), as well as in the theory regarding schizophrenia by Bleuler but mainly by Jung at the beginning of the 20th century. In 1959 Arieti and subsequently several other authors explicitly expressed the concept of psychosomatic illness or psychosomatic participation of the brain. Yet this notion tends to be put aside by the gradual prevalence, since the 17th century onwards, of the idea that the soul and brain are one and the same thing. This idea was called the “cerebral subject” by same researchers (Ehrenberg, Vidal). Due to this “ideology” the separation of neurology from psychiatry, which has been initially conceived by Georget, tends to be rejected nowadays. Restoring the concept of psychosomatic illness or psychosomatic brain participation could once again help separate these two disciplines. Psychoanalysis and new theories about neuroplasticity are of great help towards this direction – achieving restoration.Les affections psychosomatiques au sens classique sont extra-cérébrales. Néanmoins, il n’y aurait pas de raison, à priori, pour que des circuits du cerveau ne puissent pas être affectés par un tel processus. Ce type de processus, qui était sous-jacent dans les théories antiques sur le retentissement des passions, à travers les humeurs, sur le cerveau, a été évoqué implicitement aussi par les aliénistes Français du début du 19ème (Georget en particulier) et plus tard, au tournant du 20ème siècle, par Bleuler et surtout Jung dans ce qu’ils avançaient sur la schizophrénie. En 1959 Arieti puis plus tard divers autres auteurs ont exprimé explicitement le concept d’affection ou participation psychosomatique du cerveau. Mais en général cette notion tend à être refoulée par la montée en puissance, à partir du 17ème siècle, de l’idée que notre psyché équivaut à notre cerveau que certains auteurs (Ehrenberg, Vidal) ont appelé «le sujet cérébral ». A cause de cette « idéologie » la séparation entre la neurologie et la psychiatrie, qui était acquise depuis Georget, de nos jours tend à être à nouveau abolie. La réhabilitation du concept d’affection ou participation psychosomatique du cerveau pourrait aider à une « mise à jour » de la séparation de ces deux disciplines.
