2012 – 2014 years retrospective analysis of acute non – variceal upper gastrointestinal bleeding cases in the Hospital of Lithuanian University of Health Sciences.

The aim: To evaluate the most common reasons for acute non – variceal upper gastrointestinal bleeding (UGIB), treatment methods: phramacologic therapy, endoscopic therapy, interventional angiography, surgery and to rate the Rockall score prognostic value in determining the frequency of rebleeding and mortality in the Hospital of Lithuanian University of Health Sciences. The methods: Retrospectively analyzed the cases of the patients admitted to the University hospital due to non – variceal UGIB in the years 2012 – 2014. Bleeding activity was assessed according to the Forrest classification, condition – according to the Rockall score. Statistical analysis was performed using SPSS version 20.0 statistical software package. Parametric criteria assessed using Student's t test, non – parametric – χ2 test. In this research, only the results with p<0.05 were considered as statistically significant. The results: There were 390 cases: 232 (59.5 %) male, mean age – 57.9 ± 17.4 and 158 (40.5 %) female cases, mean age – 70.6 ± 16.9, p<0.001. The most common reason for non – variceal UGIB was peptic ulcer, found in 321 (82.3 %) patients, while other reasons were: Mallory – Weiss tear in 36 (9.2 %), erosive GERD – 13 (3,3 %), hemorrhagic gastritis – 11 (2.8 %), erosions – 6 (1.5 %), angiodysplasia in 2 (0.5 %) of the cases. Active bleeding (Forrest IA, IB) manifested in 122 (31.3 %) cases, not – active (Forrest IIA, IIB, IIC, III) in 268 (68.7 %) cases. Endoscopic hemostasis was applied in 265 (67.9 %) cases. Among endoscopically treated patients in 150 (56.6 %) only injections of adrenalin was used. Rebleeding was observed in 34 (8.7 %) cases. Among patients with active bleeding 11 (9.0%) patients had rebleeding, among patients with inactive bleeding 23 (8.6 %) patients had rebleeding, p=0.88. Twenty – nine (7.4%) patients died. Among patients with active bleeding 10 (8.2 %) patients died, 112 (91.8 %) – survived, among patients with inactive bleeding 19 (7.1%) patients died, 249 (92.9 %) – survived, p=0.69. Angiography was applied in 28 (7.2 %) cases, of which 8 (28.6 %) died. Surgical treatment was applied in 10 (2.6 %) cases, of which 4 (40.0 %) died. Baseline Rockall score was 5.4 ± 2.3 in patients who had rebleeding and 4.3 ± 2.2 in patients who did not rebleed, p<0.05.
