Pecularities of haemodialysis patients nursing in opinion of patients and nurses according to the data of Department of Detoxication of Kaunas University of Medicine Hospital

Goal of the work: to assess the nursing peculiarities of hemodialysis patients. Methods. The survey of respondents was performed in performing the research at Kaunas University Hospital. Two anonymous questionnaires were applied. One questionnaire was prepared for hemodialysis patients and another for nurses. The questions in the questionnaire were targeted for evaluation of social demographic data of the researched persons, for the analysis of patient knowledge on their illness, sleep, adaptation to the changed mode of life, methods of solving the health problems, communication with the nursing staff and information provided by it. The questionnaire for patients contained questions on their opinion on their health and its meaning to daily activities („Health Survey for Dialysis Patients (SF36)). While calculating the mean values of the quantitative values, the standard deviations are specified next to them. The statistic data analysis was performed by computer, by using the Statistical Package for Social Science software package. The date the difference was disregarded if the meaning level was p0.05). 74.1 percent of patients evaluated their health condition sufficiently, 7.4 percent - poorly, and the remaining - good. The most limiting factor of health for patients was their activities (74.1 percent), the least - washing themselves and dressing independently (11.1 percent). The elder age of patients, the more their health was a limit factor in different activities (r=-0.467, p<0.001). The patients who had no problems on their arrival to hemodialysis centre, statistically significantly had less limitations in their daily life in comparison to patients who have health problems (p=0.003). 85,2 percent of all patients were completely satisfied with communication with nurses, 13 percent – partially (in the opinion of nurses respectively 13,3 percent and 86,7 percent, p<0.001) and 1,9 percent (n=1) – unsatisfied. 51.9 percent of patients were considering that they do not have sufficient knowledge of their illness; on the opinion of nurses, this number was 76.7 percent (p=0.0289). In the opinion of patients, for 48.1 percent of their problems arising during their treatment in haemodialysis centre, they get most of assistance from the doctor; nurse helps for 16.7 percent of patients, and 20.4 percent of patients are often asking for help from their family and relatives. As many as 8 (14.8 percent) of patients mentioned all 3 answers. As many as 75.9 percent of patients are always eagerly discussing their problems with doctor, 50.0 percent with the nurse, 77.8 percent of them discuss all problems with their families and as many as 42.6 percent - with other patients. Statistically significantly less nurses (36.7 percent) were considering that the patients always discuss their difficulties with doctors and family (p<0.001). In their opinion, the major helping hand in their problem-solving is the nurse - 76.7 percent (doctor - 46.7 percent, p<0.05). The opinion that nurses give the sufficient explanations for patients on their illness, regime and other interesting issues, was prevailing between 64.8 percent of patients and between 43.3 percent of nurses (p=0.059). There were statistically reliable differences between the opinions of patients and nurses that nurses should have more discussions with patients on issues that are of interest to them (53.3 percent of nurses and 24.1 percent of patients, p=0.0085). The majority of the survey participants (70.0 percent of nurses and 33.3 percent of patients considered that nurse does not provide her time for conversations with patients (p<0.01). Conclusions. 1. The bigger the distance to the dialysis centre, the more problems the patients have for the arrival of the haemodialysis procedure (p=0.008). Men being treated by haemodialysis easier adapted to the new diet, liquid limitation regime and new life rhythm than women (p=0.02). 2. Three fourths of patients treated with haemodialysis consider their health is in satisfactory condition. Health of patients of elder age was more significantly limiting their daily activities, in comparison with younger patients (p<0.001). Health of haemodialysis patients that prevent their arrival to the haemodialysis procedure was also more limiting them in their daily activities in comparison to the patients for whom the arrival to the dialysis did not pose any difficulties (p=0.003). 3. Half of all haemodialysis patients always addressed the nurses in respect of their arising health problems. In case of difficulties, patients are more relying on doctors than on their family or nurses, whereas from the viewpoint of the nurses, they are the ones who play a more significant role here (p<0.05). 4.
