Analysis of outcomes after laparoscopic colorectal resection in the Hospital of Lithuanian University of Health Sciences Kauno klinikos

AIM: The aim of this study is to assess short-term outcomes after laparoscopic colorectal resection in the Hospital of Lithuanian University of Health Sciences Kauno klinikos (LSMUL KK) and compare them with literature data. OBJECTIVES: 1. To evaluate which diseases of the colon and rectum are treated with laparoscopic approach, what is the mean operative time (MOT), what are specifications of the surgical specimens, the rate of postoperative complications and the mean hospital stay (MHS). 2. To evaluate what part of the colon and rectum operations are laparoscopically performed in the LSMUL KK. METHODS: A retrospective data analysis of patients undergoing laparoscopic colorectal resection performed at the LSMUL KK Unit of Surgery from 01-01-2009 to 01-01-2015 was conducted. RESULTS: During 6 years period 67 patients were operated on laparoscopically for colorectal disorders, 26 of them were males and 37 females, with the mean of 63,1 years age. 43 patients were operated for colorectal adenocarcinoma (for stage I- 14, stage II- 15, stage III- 14 and stage IV- 1), 2 for diverticular disease, 9 for rectal prolapse and 6 for tubulovillous colon adenoma. 33 patients underwent sigmoid and 14 rectal resections, 11 right-sided and 5 left- sided colectomies. The MOT was 165min ± 36 SD, the mean length of the surgical specimen was 14,8cm ± 7,6 SD and the mean lymph node harvest was 10,4 ± 5,4 SD. The MHS was 10,5 ± 4,8 SD days. An anastamotic leak occurred in 5 (7,9%) and other complications in 6 (9,5%) cases (4- post-operative wound infections, 1- small bowel perforation, 1- clinically significant bleeding). During the period from 01-01-2009 to 01-01-2015 a total of 1783 colorectal operations were performed, 63 (3,5%) of them were operated on laparoscopically. CONCLUSIONS: 1. The most common complications after laparoscopic colorectal resection in the LSMUL KK Unit of the Surgery are anastamotic leak and post-operative wound infection. The rate of these complications is similar to other studies. 2. The mean hospital stay, compared to other studies, was longer because of absence of the Fast-Track surgery protocol. 3. The mean number of lymph nodes detected in the colorectal resection specimens is insufficient to correctly evaluate the TNM stage. 4. There is a small number of colorectal operations performed laparoscopically (3,5%). The detailed data analysis of patients who underwent colorectal operation is required to evaluate how much operations can be potentially performed laparoscopically
