Influence of the reactivation of CMV infection and CMV disease for the renal graft survival

Citomegalovirus is one of the most important causes for infection after the renal transplantation, which is associated with larger post-transplantational morbidity and mortality as well as shorter duration of renal graft survival. Therefore, the aim of this study is to estimate the frequence of reactivation of CMV infection and CMV disease after renal transplantation, and to evaluate the methods of CMV prophylaxis and other factors that have influenced renal graft survival. The objectives of this study are: to determine the CMV serological status and prescribed prophylactic treatment during the early post-transplantational period; to estimate the frequence of both CMV infection and disease for patients who underwent the cadaveric renal transplantation during the period of 2000-2014 years in LSMUL KK; to evaluate the influence of CMV infection and disease, duration of intravenous ganciclovir prophylaxis, age and causes of death of donors for the renal graft survival. Methods: the medical documentation of patients who underwent the cadaveric renal transplantation during the period of 2000-2014 years in LSMUL KK was studied, their statistical analysis was done and the influence of various factors for the graft survival was evaluated. Results: The most common anti-CMV IgG serological status of donor/recipient was D+/R+. Most of the patients underwent the post-transplantational prophylaxis by intravenous ganciclovir. CMV infection and disease were uncommon. The graft survival depends on the duration of the early prophylaxis of intravenous ganciclovir and the age of donor, when the terminal stage of the renal insufficiency developed. When the graft is not in the terminal stage of the renal insufficiency, its survival depends on anti-CMV IgG serological status of donor/recipient, the duration of the early prophylaxis of the intravenous ganciclovir, the cause and age of donor’s death and CMV infection during the ambulatory period of treatment
