Objective: Study of the immediate maternal-fetal morbidity with instrumental extractions.Patients and methods: prospective study type exposed / unexposed conducted over 10 months on 288 instrumental deliveries (142 spatulas, 53 forceps and 93 vacuum extractor) compared to 288 matching spontaneous deliveries.Results: The operative deliveries increase maternal-fetal morbidity. The rate of episiotomy, 2nd or 3rd degree lacerations, transfusions and iron supplementation are increased. Episiotomies are more common in the forceps group. Newborns of instrumental delivery group have lowest birth PH, more injuries of the scalp or bloody seroma bump (BBS), rate of birth resuscitation and rate of admission in neonatology are more important.
