The chronic care model (CCM) deployed through a learning collaborative strategy, such as the Institute for Healthcare Improvement’s Breakthrough Series (BTS), is a widely adopted approach to improve care that has guided clinical quality initiatives nationally and internationally. The BTS collaborative approach has been used to improve chronic conditions at national and state levels and in single health care delivery systems but not in correctional health care.
