Synthesis and bioevaluation of novel PET tracers for the imaging of P-glycoprotein

Die Blut-Hirn-Schranke (BHS) trägt wesentlich zur Erhaltung der Homöostase im Gehirn bei, indem sie die Zufuhr von Nährstoffen reguliert und als Schutz vor potenziell schädlichen Einflüssen fungiert. Diese Schutzfunktion wird unter anderem durch die Expression von Adenosintriphosphat-Binding-Cassette (ABC) Transportern wie P-Glycoprotein (P-gp), Multidrug resistance-related-Proteinen (MRP) und Breast cancer-resistance-Protein (BCRP) erfüllt. Diese Transporter sind in der Lage eine breite Auswahl an Substanzen, wie z.B. Arzneistoffe, aus dem Gehirn zu transportieren. Es gibt Hinweise, dass Änderungen in der Expression der ABC Transporter eine bedeutende Rolle bei der Ätiologie und Resistenzentwicklung diverser neurologischer Erkrankungen wie Epilepsie, Alzheimer und Parkinson spielen. Die Methode der Positronen-Emissions-Tomographie (PET) könnte dazu verwendet werden um in vivo im Patienten ABC Transporter an der BHS zu untersuchen. Daher wurden PET-Tracer wie beispielsweise (R)-[11C]Verapamil oder [11C]N-Desmethyl-loperamid entwickelt, die es erlauben die funktionelle Aktivität von P-gp an der BHS zu messen. Diese PET-Tracer zeigten jedoch nur sehr geringe Gehirnaufnahme und sind für ein Mapping der P-gp-Expression ungeeignet. Aus diesem Grund wurde in dieser Arbeit der Versuch gestartet neuartige PET-Tracer zur Visualisierung der P-gp Expression an der BHS zu entwickeln, die, anders als vormals vorhandene Tracer, nicht auf hochaffinen Substraten wie Verapamil oder N-Desmethylloperamid, sondern auf P-gp-Inhibitoren der dritten Generation beruhen. O-Desmethylderivate der beiden Tracerkandidaten Tariquidar und HM30181 wurden in sechsstufigen Synthesen, in Gesamtausbeuten von 26 % beziehungsweise 16 %, als Vorläufermoleküle zur Radiomarkierung synthetisiert. Diese wurden im Anschluss mit [11C]Methyltriflat zu [11C]Tariquidar und [11C]HM30181 umgesetzt. Die so gewonnenen PET Tracer wurden in einer Bioevaluierung in Ratten und Transporter-defizienten Mäusen getestet. Es zeigte sich, dass beide Tracer nur eine geringe Aufnahme ins Gehirn aufweisen und sich in vivo wie duale P-gp/BCRP Substrate verhalten. Für eine Anwendung als Tracer zur Messung der regionalen Verteilung von P-gp an der BHS sind beide Verbindungen daher ungeeignet. Dennoch ist vor allem [11C]Tariquidar ein PET-Tracer der näher untersucht werden sollte, da weiterführende Experimente eine Eignung als Marker für die Darstellung der P-gp Expression in soliden Tumoren beziehungsweise zur Messung der funktionellen Aktivität von P-gp und BCRP an der BHS nahelegten.The blood-brain barrier (BBB) maintains homeostasis in the brain by regulating the supply of nutrients and by protecting the brain from potential harmful influences. This protective function is, amongst other factors, maintained by the expression of adenosine triphosphate binding cassette (ABC) transporters, such as P-glycoprotein (P-gp), multidrug resistance-related proteins (MRPs) and breast cancer resistance protein (BCRP). These transporters are capable of transporting a broad variety of substances including drugs out of the brain. There is evidence that changes in the expression of ABC transporters play an important role in the pathophysiology and in the development of drug resistance in different neurological disorders including epilepsy, Alzheimer’s and Parkinson’s disease. The non-invasive imaging method positron emission tomography (PET) can be used to examine ABC transporters at the BBB in patients in vivo. PET tracers such as (R)-[11C]verapamil and [11C]N-desmethyl loperamide were developed which allow for measuring the functional activity of P-gp at the BBB. However, these radiotracers possess low brain uptake and are therefore not suitable for mapping the expression of P-gp. In this thesis it was attempted to develop novel PET tracers for the visualization of P-gp expression, which, unlike earlier developed probes, are not based on high-affinity substrates like verapamil and N-desmethyl loperamide, but on third generation P-gp inhibitors. O-Desmethyl derivatives of the tracer candidates tariquidar and HM30181 were synthesized in six-step syntheses, with overall yields of 26 % and 16 %, respectively, as precursors for radiolabelling. These precursors were subsequently reacted with [11C]methyl triflate to obtain [11C]tariquidar and [11C]HM30181. The newly developed PET tracers were evaluated in rats and in transporter-deficient mice. Contrary to initial expectations both radiotracers possessed low brain uptake, which was due to efflux transport by P-gp and BCRP. Therefore, both radiotracers are not suited to measure the regional distribution of P-gp at the BBB. Nevertheless, [11C]tariquidar merits further investigation, as additional experiments suggested that this tracer might be suited for imaging the expression of P-gp in solid tumours and for measuring the functional activity of P-gp and BCRP at the BBB
