Homeless persons diagnosed with severe mental illness represent approximately a quarter of the homeless population in the United States. The risk of deteriorating health is increased the longer a person with severe mental illness remains on the streets. Supportive Housing Model (SHM) was introduced in the early 1990’s to address this public health concern. SHM intended to provide affordable permanent housing, with added support provided by case managers, social workers, and various treatment providers to assist residents in accessing services to help lengthen their time away from homelessness.
