Treatment results of adiuant and neoadiuant radiation therapy for rectal cancer patients

Kolorektālais vēzis ir pasaulē trešā biežākā vēža lokalizācija vīriešiem un otra biežākā sievietēm, turklāt tas ir ceturtais biežākais nāves iemesls no kopējā audzēju skaita.Ik gadu Latvijā tiek diagnosticēti vairāk kā 1000 jaunu kolorektālā vēža gadījumu. Pēdējos 70 gadu laikā kolorektālā vēža ārstēšana ir attīstījusies no tikai ķirurģiskas līdz kombinētai terapijai,kas sevī iekļauj ķirurģiju, staru terapiju un ķīmijterapiju. Pareiza adjuvantās un neoadjuvantās terapijas plānošana un savlaicīga piemērošana ievērojami samazina lokālā recidīva un attālo metastāžu risku taisnās zarnas vēža pacientiem. Šī pētījuma mērķis ir novērtēt adjuvantas un neoadjuvantas staru terapijas efektivitāti taisnās zarnas vēža ārstēšanā.Materiāls un metodes. Šis ir retrospektīvs pētījums.Darbā analizēta adjuvantas un neoadjuvantas staru terapijas ietekme uz ārstēšanas rezultātiem taisnās zarnas vēža pacientiem, kuriem ir veikta radikāla operācija. Pētījuma populācija ir 2007. gadā Latvijas Onkoloģijas Centrā radikālu ķirurģisku ārstēšanu saņēmušie taisnās zarnas vēža pacienti, kuriem ir bijusi piemērota adjuvanta vai neoadjuvanta staru terapija.Secinājumi. Neoadjuvanta staru terapija ievērojami samazina lokāla recidīva un attālu metastāžu parādīšanās biežumu.Background.Colorectal cancer is the second most common malignancy in females and third most common malignancy in males in Latvia. Every year there are 1000 new cases of colorectal cancer in Latvia. In last 70 years treatment for rectal cancer has evolved from surgery alone to combined three modality therapy. Since the local relapse rates for surgery alone are up to 50% planning and optimal use of adiuvant or neoadiuvant therapy is crucial in achievment of good results in rectal cancer treatment. Radiation therapy, chemotherapy or both can be added as adiuvant or neoadiuvant treatment prior to surgery or postoperatively to enhance the outcomes of radical surgery. The aim of this study is to compare efficiency of adiuvant and neoadiuvant radiation therapy for histologicaly veriffied rectal cancer patients who have been exposed to radical surgery.Population of this study are the patients who have recieved adiuvant or neoadiuvant radiation therapy and have been exposed to radical surgery in the period of Year 2007 in Oncology Center of Latvia. Materials and methods. This is a retrospective study. There were total of 68 patients. Patients were divided into two groups, recieving either adiuvant or neoadiuvant radiation therapy. The treatment results were analised depending on stage of tumor,addition of chemotherapyand surgery technique used. Conclusions. Neoadiuvant radiation therapy signifficantly increases local control and decreases incidence of distant metastases
