The quality of health care in care home geriatric  patients

Veselības aprūpes reformas Latvijas Republikā ir ļāvušas paplašināties veselības aprūpes sistēmai, jo tās galvenais mērķis- valsts iedzīvotājiem pieejamu veselības aprūpi- tādēļ aktuālāks paliek piedāvājums par veselības aprūpi mājās. Sabiedrības novecošanas situācijā, geriatriskiem pacientiem tas ir būtisks un pieejams pakalpojums. Pastāv uzskats, ka kvalitatīvu veselības aprūpi var sniegt tikai ārstniecības iestādēs. Tomēr arvien vairāk, pakalpojumu izmantojušie pacienti, atsaucas uz labu un kvalitatīvu aprūpi. Veicot pētījumu, liela nozīme tika likta uz aprūpes māsu kompetenci, uz viņu zināšanām, prasmi izmantot teorētiskās zināšanas, kuras balstās uz V.Hendersones pamatvajadzību teorijas un uz aprūpes procesa svarīgo un ciklisko vērtējumu. Darba autore izvēlējās pētījumu, kura ietvaros varētu uzzināt geriatrisko pacientu apmierinātību ar aprūpi mājās. Pētījuma gaitā tika izvrzīti divi kvalitātes kritēriji- veselības aprūpe mājās un aprūpes māsu kompetence. Bakalaura darbam izvirzīja tēmu- veselības aprūpes kvalitāte aprūpē mājās geriatriskiem pacientiem. Tēmas aktualitāti nosaka sabiedrības mazā informētība par veselības aprūpi mājās, geriatrisko pacientu psiholoģiskie traucējumi, saskaroties ar dažādām izmaiņām savā apkārtējā vidē, neizpratne par aprūpes māsu darbību un zināšanu izmantošanu savu prakšu laikā. Darba mērķis ir aprakstīt gan teorētiskājā daļā, gan empīriskā pētijuma gaitā veselības aprūpes kvalitāti veselības aprūpē mājās geriatriskiem pacientiem. Pētniecības uzdevumi ir izmantojot zinātnisko literatūru un citus informācijas avotus iepazīties ar veselības aprūpi mājās un izvirzīt kvalitātes kritērijus. Kā otrais uzdevums tika izvirzīts raksturot veselības aprūpes kvalitātes kritērijus veselības aprūpē mājās geriatriskiem pacientiem. Trešais uzdevums palīdzēs izvērtēt veselības kvalitātes kritēriju pielietojumu veselības aprūpē mājās geriatriskiem pacientiem. Tika izvirzīta pētījuma hipotēze- izmantojot veselības aprūpi mājās, geriatriskie pacienti saņem kvalitatīvu veselības aprūpi. Pētijuma gaitā, teorētiskam pētījumam izmanto zinātniskās literatūras un informatīvo avotu analīzi. Pētījuma empīriskai daļai sastādīja anketu, kurā izmanoja Likerta 5- punktu mērījuma skalu. Iegūtos rezultātus analizē izmantojot kontentanalīzi, lai izmērītu respondentu labsajūtas un apmierinātību, un aprakstošo statistiku, lai iegūtu priekšstatu par skiatliskiem mērījumiem respondentu apmierinātībai. Anketas aizpildīja astoņdesmit divi respondenti vecumā no 60 līdz astoņdesmit gadiem, vientuļi un ģimenes un tuvinieku palīzdību saņēmēji. Respondenti izmanto gan valsts apmaksātu, gan maksas veselības aprūpi mājās. Pētījuma rezultātā darba autore ieguva rezultātus, kuri apstiprināja izvirzīto hipotēzi, ka veselības aprūpe mājās sniedz kvalitatīvu vesel’;ibas aprūpi. Kā norādīja respondenti savās anketās, neviens, atbildot uz jautājumu par apmierinātību, neatzīmēja negatīvi tendētu atbildi. In the situation of population aging, this is an essential service for geriatric patients. Common perception is that good quality health care can be provided only by medical institutions, however patients who use the service are becoming more and more interested in the quality of the health care disregarding the instance or person who offers it. Large part of the study was focused on the competency of nurses, their knowledge and ability to use theoretical knowledge, which is based on basic needs theory of V. Henderson and the health care process and cyclical analysis. Therefore, the author has chosen to study geriatric patient satisfaction with health care at home. During the research, two quality criteria have been chosen – the quality of health care and competency of nurses. The topic of the study was chosen to be – the quality of health care at home for geriatric patients. The importance of the study is due to limited public awareness of health care at home, psychological instability of geriatric patients when facing variety of changes in their environment, lack of understanding of nurse performance and knowledge in their practice. The purpose of the study is to describe and empirically assess the quality of health care at home for geriatric patients. Research objectives are to familiarize with health care at home and to set quality criteria using scientific literature and other information sources. The second task was set to describe the health care quality criteria for health care at home for geriatric patients. The third task will assess the use of health quality criteria for health care at home for geriatric patients. The hypothesis of the study was chosen – by using health care at home, geriatric patients receive qualitative health care. For the theoretical study of the research, scientific literature and information source analysis has been used. Empirical part of the study comprised a questionnaire with Likert 5 - point measurement scale. The results obtained were analyzed using content analysis to measure the well-being and satisfaction of respondents and descriptive statistics, to get an overview of numerical measurements of satisfaction of the respondents. Questionnaires were completed by eighty two respondents aged between 60 and 80 years of age, single persons and patients who receive care from family and relatives. Respondents are using both state-funded and paid service care at home. As a result of the study, the author obtained results confirming the proposed hypothesis that health care at home provides qualitative health care. As stated by respondents in their questionnaires, no response to the question on satisfaction was marked with negative answer. Keywords: home care, nurs
