Moyamoya syndrome is a disease of children and young adults caused by fibromuscular dysplasia of the internal carotid artery. We report an autopsy case of a 6-year-old boy with moyamoya disease who presented with hypertension associated with unilateral renal artery stenosis. Intimal thickening with wavy internal elastic lamina of carotid, coronary and renal arteries were observed. This is a rare case of moyamoya disease which proves histopathologically that hypertension in this case was induced by renal artery stenosis
