Discharge planning as a tool for quality of health education.

The patient's discharge plan is multi-faceted. It is the process by which coordination between health care providers and patients and their families to provide for continuing care for patients. It was found that patient working out plans to reduce the cost of patient maintaining in the hospital and let the best take advantage for beds and emphasis on the exploitation of environmental resources to meet the needs of the patient. It is also considered as a mean (tool) for information documentation that will be taught to the patient as well as a result of this education. The present study aims at: 1) Assessment of knowledge and skills of people with diabetes and to identify sources, 2) design and implement a plan out as a tool for the quality of health education for diabetics, 3) prepare and implement a training program for nurses who are caring for people with diabetes on how to prepare and implement a plan out, and 4) assessment of knowledge and skills of patients when they go out the effect of using a plan out by nurses. Study sample included the research team, who works for the nursing units, the venue of the research (17 hospital nurse Al-Moasat 17 hospital nurse university students). Above-mentioned diabetics who are stay in the units for more than 3 months (43 Al-Moasat hospital, 24 University students Hospital) as well as patients who were entered in the same units in Al-Moasat hospital for three months after the implementation of the program (40 patients). Results illustrated that the information of nurses and patients university students hospital were best than patients and hospital nurses in Al-Moasat hospital, so there was a need for the implementation of the training program to improve Al-Moasat hospital to the information and skills of nurses regarding care of diabetics and the importance of action plan out for the patient. The study recommended the following: 1. Action plan must be started for patient exit from the beginning of entering the hospital until he had got out. 2. Support head nursing staff in each hospital on the use of exit plan for patients because the increase in information given to the patient will be reflected on their education and application of information, and thus prevents the occurrence of complications for the patient and prevent patient entry repeat to the hospital. Furthermore, there is a need to support hospital manager for planning tool application to be carried out by nurses to ensure the best quality of health education
