Objective: To describe the effect of home care nursing in knowledge, hospitalization and blood pressure levels in patients with hypertension treated impaired. Method: quantitative research with a retrospective design, made from the database extension project developed in a basic health unit of a municipality in the metropolitan region of Curitiba-PR. The activities included in the project were educational group and follow-up at home for 12 months. The sample consisted of 16 users with hypertension. Results: There was a higher frequency of women with a mean age of 63 year old.
