Acostumbrarse a las barreras: estudio cualitativo de las barreras del sistema de salud colombiano para el diagnóstico y tratamiento oportuno de cáncer de mama

The current heath care system in Colombia has within it many barriers that delay the timely diagnosis and treatment of breast cancer. Most women do not recognize these barriers nor do they understand that they can be changed. They still lack the basic education to identify these barriers and to navigate the system so they can receive the quality health care they deserve. We call this phenomenon "normalization of health care system barriers." Our own study seeks to understand both real and perceived barriers that hinder women who live in Medellin, Colombia, from receiving timely breast cancer diagnosis and treatment. This is qualitative study utilizes the STRAUSSian grounded theory approach, in which 13 women between the ages of 24 years and 77 years were interviewed. All participants came from different socio-cultural and economic backgrounds, according to maximum variation criteria from PATTON. The data were analyzed using Atlas.ti software. The analysis identified two phenomena: 1. the existence of barriers in the complex Colombian health care system; 2. the willingness of the women interviewed to accept this lower level of health care as normal. We urge the women to become empowered to challenge the "status quo" in order to save their own lives and the lives of countless others through early diagnosis and treatment. Once this paradigm of the "normalization of health care system barriers" is broken down, these women could improve their quality of life by being able to make wiser choices about successfully navigating the current health care system and being able to select health care providers themselves.Das kolumbianische Gesundheitssystem zeichnet sich durch wesentliche Barrieren in Bezug auf die schnelle Diagnose und Behandlung von Brustkrebs aus. Den meisten Frauen sind diese Barrieren nicht bewusst, sie sehen auch keine Veränderungsmöglichkeiten. Es fehlt ihnen an grundlegender Bildung, die Voraussetzung für die Identifikation dieser Einschränkungen wäre und dafür, individuelle Optionen zu erkennen und zu nutzen, um sich in diesem System so zu bewegen, dass sie die Fürsorge erhalten könnten, die sie benötigen. Wir bezeichnen dieses Phänomen als Prozess der "Normalisierung von Barrieren im Gesundheitssystem". In unserer eigenen Studie haben wir nachzuvollziehen versucht, welche faktischen und wahrgenommenen Barrieren verhindern, dass Frauen aus Medellin, einer kolumbianischen Großstadt, in den Genuss einer frühzeitigen Brustkrebsdiagnose und -behandlung kommen. Hierzu haben wir 13 Frauen zwischen 24 und 77 Jahren im Rahmen eines STRAUSS folgenden Grounded-Theory-Ansatzes interviewt. Im Sinne des Konzeptes der maximalen Variation nach PATTON haben wir Frauen mit sehr unterschiedlichem soziokulturellen und ökonomischen Hintergrund in unsere Untersuchung einbezogen. Für die Datenanalyse kam Atlas.ti zum Einsatz. Es konnten zwei Phänomene identifiziert werden: 1. die Existenz von Barrieren im kolumbianischen Gesundheitssystem; 2. die Bereitschaft der Frauen, die wir interviewt haben, die schlechtere Gesundheitsversorgung als normal hinzunehmen. Vor diesem Hintergrund halten wir es für dringend erforderlich, darauf hin zu drängen, dass diese Frauen in einer Weise ermächtigt werden und sich selbst ermächtigen, dass sie den "Status quo" infrage stellen, um ihr eigenes Leben und das zahlloser anderer durch frühzeitige Brustkrebsdiagnose und -behandlung zu retten. Nur wenn dieser Prozess der "Normalisierung von Barrieren im Gesundheitssystem" aufgebrochen wird, haben diese Frauen eine Chance, die eigene Lebensqualität zu verbessern, indem sie sich selbstbewusster und erfolgreicher in diesem Gesundheitssystem bewegen und selbst die Art von Versorgung wählen können, die sie benötigen.El actual sistema de de salud colombiano contiene muchas barreras que retrasan el diagnóstico oportuno y el tratamiento de cáncer de mama. Muchas mujeres no reconocen estas barreras o no entienden que pueden cambiarlas. Ellas piensan que el sistema de salud es inmodificable, las mujeres todavía carecen de la educación básica para identificar y navegar por el sistema de salud y superar las barreras inherentes, para que ellas puedan recibir la atención médica de calidad que merecen. Nosotras llamamos a este fenómeno "normalización de las barreras del sistema de salud". Nuestro estudio busca, comprender las barreras reales y percibidas que impiden a las mujeres que viven en Medellín, Colombia recibir el diagnóstico y tratamiento oportuno para el cáncer de mama. Se trata de un estudio cualitativo, que utiliza el enfoque Straussiano de la Teoría Fundamentada, en el que participaron 13 mujeres, entre 24 y 77 años de edad. Todas las participantes provenían de diferentes condiciones socioculturales y económicas, de acuerdo a los criterios de máxima variación de PATTON. Los datos fueron analizados utilizando el software Atlas.ti. El análisis identificó dos fenómenos: 1. la existencia de barreras en el complejo sistema de salud colombiano y, 2. la voluntad de las mujeres entrevistadas para aceptar un nivel más bajo de atención de salud como algo normal. Nosotras instamos a que las mujeres se empoderen y desafíen el "status quo" con el fin de salvar sus propias vidas y las vidas de muchas otras mujeres a través del diagnóstico y tratamiento temprano. Una vez que este paradigma de la "normalización de las barreras del sistema de salud" se rompa, las mujeres podrán mejorar su calidad de vida al ser capaces de tomar decisiones más sabias acerca de cómo navegar con éxito en el sistema de salud y ser capaces de seleccionar los proveedores de atención de salud por sí mismas
