Prevalence and severity of cornea guttata in the graft following Descemet Membrane Endothelial Keratoplasty (DMEK)

Objective: The aim of this study was to determine the occurrence and severity of a corneal guttata over time after Descemet Membrane Endothelial Keratoplasty (DMEK) and to investigate the impact on various clinical parameters. Patients and Methods: The retrospective study examined a total of 664 corneal transplantations on 633 eyes between 2013 and 2019. The frequency and the time course of the guttae after the operation were examined by using specular microscopic images. The severity was classified according to 4 previously defined grades (G0 (no guttae), G1-G3 with increasing severity). Various clinical parameters of routine postoperative examinations, such as visual acuity, corneal thickness, endothelial cell density, pleomorphism and polymegalism were analyzed at specific time intervals (6weeks, 6months, 12 months and then annually). Results: In 124 (18.7%) of the 664 operated eyes a corneal guttata could be detected immediately postoperatively in the first control examination. In 16.9% a mild grade G1 was most common and only 1.8% of the grafts showed a higher grade G2 or G3. In the clinical examination, differences could be shown especially between G0/G1 and the higher grade stages G2/G3. A significant decrease of the best corrected visual acuity could be shown between G1 and G3 (p=0.03). Central pachymetry showed an increase of corneal thickness between G0 and G1 to G3 (p<0.01/p=0.04). In addition, a statistically significant decrease of endothelial cell density (p<0.001), hexagonal cells (p<0.01) and a significant increase of the cell variation coefficient (p=0.04) could be detected with increasing guttae grades. Conclusion: In 18.7% of the grafts a corneal guttata could already be detected in the first examination immediately after transplantation. This suggests that the guttae could have affected grafts already preoperatively and could not be detected beforehand despite the strict quality controls in the eye bank. However, the study also found that 90% of affected grafts had only a mild form of corneal guttata (G1), which had no relevant impact on the functional and morphological outcomes of the transplantation. Nevertheless CG grades G2 and G3 relevantly influenced postoperative outcomes after DMEK.Fragestellung: Ziel dieser Studie war es, das Auftreten sowie den Schweregrad einer Cornea guttata im zeitlichen Verlauf nach Descemet Membran Endothelial Keratoplasty (DMEK) zu ermitteln und den Einfluss auf die klinischen und morphologischen Parameter zu erfassen. Patienten und Methode: In der retrospektiven Studie wurden im Zeitraum von 2013 bis 2019 insgesamt 664 Transplantationen an 633 Augen untersucht. Mittels der postoperativen spiegelmikroskopischen Kontrollaufnahmen wurden die Häufigkeit, der Schweregrad und der zeitliche Verlauf der Guttae ermittelt.
