Oxidative stress in preeclampsia: Relationship between lipoprotein oxidation and paraoxonase enzyme activity

Preeklampsi; gebeliğin 20. haftasından sonra görülen proteinüri ve/veya ödemin eşlik ettiği arteriyel kan basıncında yükselmeyle karakterize multi sistemik bir bozukluktur, oluşum mekanizması açıklığa kavuşmamıştır. Bu çalışmaya 29 sağlıklı gebe, 24 hafif preeklampsi ve 24 ağır preeklampsi vakası alındı. Hafif preeklampsili gebelerin 13' ü ve ağır preeklampsili gebelerin 17' sinde doğum sonrasında da ölçümler tekrarlandı. Her iki preeklampsi grubunda serum ürik asit, trigliserit, apolipoprotein B, malondialdehit ve ağır preeklampsi grubunda lipoprotein (a) düzeylerinin sağlıklı gebelere göre yüksek olduğu bulundu. Ayrıca, preeklampsili gruplarda eritrositlerin ve apolipoprotein B-içeren lipoproteinlerin oksidasyona duyarlılıklarında artış olduğu gözlendi. Hafif ve ağır preeklampsili grupların, eritrosit glutatyon peroksidaz, süperoksit dismutaz aktivitelerinin ve serum total antoksidan kapasitelerinin sağlıklı gebelere göre anlamlı derecede düşük; serum katalaz aktivitesinin ise yüksek olduğu bulundu. Serum paraoksonaz ve arilesteraz aktivitelerinin anlamlı farklılık göstermediği saptandı. Serum total karoten düzeylerinin her iki preeklampsili grupta sağlıklı gebelere göre daha düşük olduğu bulundu. Preeklampsili grupların doğum sonrası verileri incelendiğinde, her iki grupta da doğum sonrası ürik asit, total kolesterol, trigliserit, apolipoprotein Al, apolipoprotein B düzeylerinde anlamlı azalma olduğu; total protein ve albümin düzeylerinde ise artış olduğu saptandı. Doğum sonrası dönemde serum malondialdehit ve eritrositlerin oksidasyona duyarlılıklarında anlamlı azalma saptandı, apolipoprotein B-içeren lipoproteinlerin oksidasyona duyarlılıklarında ise anlamlı farklılık olmadığı gözlendi. Her iki preeklampsili grubun total antioksidan aktiviteleri ve hafif preeklampsili grubun eritrosit süperoksit dismutaz aktivitesi doğum sonrası dönemde anlamlı artış gösterdi, paraoksonaz ve arilesteraz aktivitelerinde ise anlamlı bir farklılık saptanmadı. Sonuç olarak bu çalışmanın verileri preeklampside oksidan - antioksidan sistem arasındaki dengenin bozularak oksidatif stres tablosunun açığa çıktığını ve oksidatif stresin patogenezde önemli rolü olduğunu destekler niteliktedir.Preeclampsia is a multisystemic disorder characterised by elevated arterial blood pressure accompanied with proteinuria and/or oedema. The pathophysiology of preclampsia has not been clearly understood. Twenty nine healthy pregnant women and 24 pregnant women with mild and 24 pregnant women with severe preeclampsia were included in this study. Thirteen women from the mild preeclamptic group and 17 from the severe preeclamptic group were also studied in the postpartum period. Uric acid, triglyceride, apolipoprotein 8, malondialdehyde levels were significantly higher in both of the preeclamptic groups and lipoprotein (a) levels were found to be significantly higher in the severe preeclamptic group compared with the healthy pregnant group. Oxidizability of apolipoprotein B-containing lipoproteins and erythrocytes were enhanced in both of the preeclamptic groups compared with the healthy pregnant group. Erythrocyte glutathione reductase and superoxide dysmutase activities, serum total antioxidant capacity were significantly lower and serum catalase activity was significantly higher in both of the preeclamptic groups than in the healthy pregnant group. There were no significant differences in serum paraoxonase and arylesterase activities between the preeclamptic groups and the healthy group. Serum total carotenoids were significantly lower in the preeclamptic groups compared with the healthy pregnant group. Uric acid, total cholesterol, triglyceride, apolipoprotein Al, apolipoprotein B levels were significantly decreased and total protein and albumin levels were significantly increased in the postpartum period. Serum malondialdehyde levels and susceptibility of erythrocytes to oxidation were significantly reduced, however, there were not any differences in oxidizability of apolipoprotein 8-containing lipoproteins in the postpartum period. Total antioxidant capacity increased in both preeclamptic groups and superoxide dysmutase activity increased in the pregnants with mild preeclampsia in the postpartum period. There were not any significant changes in serum paraoxonase and arylesterase activities in the postpartum period. In conclusion, our results supported the hypothesis that preeclampsia is associated oxidative stress which is a result of imbalance between oxidant and antioxidant systems
