Effect of racial inequality on the association between socioeconomic factors and oral health throughout life

Studies have demonstrated the effect of race/skin color on inequalities in oral health. However, most of these studies still attribute this effect to socioeconomic inequalities existing in the Brazilian population. Therefore, the objective of this study was to evaluate the effect of racial inequality on the association between socioeconomic factors and subjective (self-perceived oral health) and objective (tooth loss) outcomes in oral health among participants in the 1982 Pelotas Birth Cohort. The 1982 Cohort is a longitudinal study, with several follow-ups in childhood, adolescence and adulthood that allow different studies on the health of these individuals. For this study, perinatal data, general follow-up at 30 years of age and oral health at 31 years of age were used. The main exposition is the self-declaration of race/color. The covariates used were gender, perinatal income, income at age 30, education at age 30 and social mobility (from birth to adulthood). The outcomes were self-perception of oral health answered by the participants in a questionnaire, and tooth loss assessed by a trained and calibrated dental surgeon using the DMFT index (Decayed, Missing and Filled Teeth). The statistical analyzes carried out were the absolute index of inequality (SII), concentration index (CIX), log binomial models with prevalence ratios (PR) and the “Synergy Index” (S). The prevalence of negative self-perception of oral health was 36.1% and tooth loss 50.8%. A prevalence of negative self-perception of oral health was observed in blacks/Browns/Indigenous people with higher income/education comparable to the prevalence of the outcome in Whites with lower income/education. Higher prevalence for tooth loss was found for Blacks/Browns and an interaction between race and social mobility in the never poor category (S=0.20 [CI95% 0.04-0.99]). Racial inequalities in both self perceived oral health and tooth loss persist or increase in wealthier social strata, suggesting an effect of structural and interpersonal racism on oral health outcomes.Coordenação de Aperfeiçoamento de Pessoal de Nível Superior - CAPESEstudos tem demonstrado o efeito da raça/cor da pele sobre as iniquidades em saúde bucal. Entretanto, a maior parte desses estudos ainda atribui esse efeito às desigualdades socioeconômicas existentes na população brasileira. Com isso, o objetivo desse trabalho foi avaliar o efeito da desigualdade racial na associação entre fatores socioeconômicos e desfechos subjetivo (autopercepção de saúde bucal) e objetivo (perda dentária) em saúde bucal nos participantes da Coorte de Nascimentos Pelotas de 1982. A Coorte de 1982 é um estudo longitudinal, com a realização de diversos acompanhamentos na infância, adolescência e idade adulta que possibilitam diferentes estudos sobre a saúde destes indivíduos. Para este estudo foram utilizados dados do perinatal, acompanhamento geral aos 30 anos e de saúde bucal aos 31 anos. A exposição principal é a autodeclaração de raça/cor. As covariáveis utilizadas foram sexo, renda no perinatal, renda aos 30 anos, escolaridade aos 30 anos e mobilidade social (do nascimento a fase adulta). Os desfechos foram a autopercepção de saúde bucal respondida pelo participante em um questionário, e a perda dentária avaliada por um cirurgião-dentista treinado e calibrado utilizando o índice CPO D (Dentes Cariados, Perdidos e Restaurados). As análises estatísticas realizadas foram o índice absoluto de desigualdade (slope index of inequality SII), índice de concentração (concentration index-CIX), modelos log- binomiais com razões de prevalência (RP) e o “Synergy Index” (S). A prevalência de autopercepção negativa de saúde bucal foi de 36,1% e de perda dentária 50,8%. Observou-se uma prevalência de autopercepção negativa da saúde bucal em Pretos/Pardos/Indígenas de maior renda/escolaridade comparável à prevalência do desfecho nos Brancos de menor renda/escolaridade. Para a perda dentária uma maior prevalência foi encontrada para Pretos/Pardos e uma interação entre raça e mobilidade social na categoria nunca pobre (S=0,20 [IC95% 0,04-0,99]). As desigualdades raciais tanto para autopercepção de saúde bucal quanto para a perda dentária persistem ou aumentam nos estratos sociais mais ricos, sugerindo um efeito do racismo estrutural e interpessoal nos desfechos de saúde bucal
