Définition des endotypes de l'asthme sévère chez l'enfant au travers d'une approche intégrée

Asthma is a chronic inflammatory disease that results from an excessive immune response in the respiratory mucosa associated with a remodelling of the respiratory tract. Severe asthma (SA) is characterised by the occurrence of exacerbations despite medications. SA affects 5% of asthmatic children, and remains a heterogeneous disease whose severity, frequency of symptoms and triggering factors show high inter-individual heterogeneity. It is therefore necessary to develop personalized medicine, which requires the precise definition of each clinical phenotype and the understanding of the associated endotypes. Therefore, we aimed to define the local signature of SA in children and to understand the pathophysiological mechanisms involved, as well as the associated barrier defects, by combining different approaches.First, we analysed and then compared the composition of the microbiota (16S rRNA sequencing) and the global metabolome (LC-HRMS) in bronchoalveolar lavages (BALs) collected from children suffering from SA (n = 20) and non-asthmatic children (NA, n=10) (CLASSE cohort). We then used approaches to integrate our data blocks, with cytokine data obtained previously on the same samples. We observed an increase in microbiota diversity in children with SA compared to NA, with significantly higher relative abundances of 5 bacterial genera in the BALs of SA children. In parallel, the enrichment analysis of metabolic pathways highlighted the importance of the polyamine biosynthesis pathway in SA. Unsupervised integration of multi-block data allowed the identification of a signature of the SA, composed primarily of metabolites and cytokines.Secondly, we aimed to understand the mechanisms mediated by the loss of function of the airway epithelial barrier in SA. Thanks to samples from SA and NA patients collected within a new cohort (SevAsthma-Children, cohort under construction, ANR-18-CE14-0011), we have set up banks of: i) of nasal and bronchial epithelial cells, and ii) bacteria isolated from BALs and characterised by the production of short chain fatty acids and resistance to antibiotics. A cellular model of type T2 inflammation using our SA or NA epithelial cells in the presence of IL-4 has been set up, to analyse different defence functions of the epithelium in response to this inflammation, such as ion transport, and expression of mucins and alarmins. I, in the context of development of new therapeutic approaches, we examined the impact of a Lactobacillus strain in this model of inflammation, a strain already described as having an immunomodulatory potential in lung diseases. Preliminary results suggest that IL-4 decreased epithelial sodium channel activity and increased CFTR chloride channel activity. Interestingly, the bacterium seemed to restore the control activity of the latter.My work contributes to the better characterisation of a local signature of severe asthma in children. Specifically, our multi-omics analysis highlighted a close relationship between metabolomic and immune signatures, while identifying differentially regulated metabolites and bacterial genera in the lungs of children with SA. These original results will be confirmed on the larger SevAsthma cohort, ongoing in both inclusion and follow-up. Our in vitro epithelial model of T2 inflammation has also established the basis for functional tests that will allow the evaluation of the probiotic potential of bacterial strains in the pathology of asthma.L’asthme est une pathologie inflammatoire chronique qui résulte d’une réponse immunitaire inflammatoire excessive au niveau de la muqueuse respiratoire, associée à un remodelage des voies respiratoires. L’asthme sévère (AS) est caractérisé par la survenue d’exacerbations malgré un traitement de fond. Il touche 5% des enfants asthmatiques, et reste une maladie hétérogène dont la sévérité, la fréquence des symptômes et les facteurs déclenchants sont très variables en fonction des individus. Il est donc nécessaire de développer une prise en charge adaptée à chaque patient, ce qui nécessite la définition précise de chaque phénotype clinique et la compréhension des endotypes associés. Dans cette thèse, nous avons donc chercher à définir la signature locale de l’AS sévère chez l’enfant et d’en comprendre les mécanismes physiopathologiques, ainsi que les défauts de barrière associés, en combinant différentes approches.Dans un premier temps, nous avons analysé puis comparé la composition du microbiote (séquençage de l’ARNr 16S) et du métabolome global (LC-HRMS) dans les lavages bronchoalvéolaires (LBA) collectés chez des enfants souffrant d’AS (n=20) et des enfants non-asthmatiques (NA, n=10) (cohorte CLASSE). Nous avons ensuite utilisé des approches permettant d’intégrer nos blocs de données, en incluant également des données de cytokines préalablement obtenues sur ces mêmes échantillons. Nous avons observé une augmentation de la diversité du microbiote chez les enfants souffrant d’AS par rapport aux NA, avec des abondances relatives significativement plus élevées de 5 genres bactériens dans les LBA des enfants AS. En parallèle, l’analyse d'enrichissement des voies métaboliques a mis en évidence l’importance de la voie des polyamines dans l’AS. L'intégration non-supervisée de données multi-blocs a permis d’identifier une signature de la pathologie, principalement composée de métabolites et de cytokines.Dans un second temps, nous avons cherché à comprendre les mécanismes contribuant à la perte de fonction de la barrière épithéliale des voies respiratoires dans l'AS. Pour cela, grâce à des échantillons de patients AS et NA collectés au sein d’une nouvelle cohorte (SevAsthma-Children, en cours de constitution, ANR-18-CE14-0011), nous avons constitué des banques : i) de cellules épithéliales nasales et bronchiques, et ii) de bactéries isolées de LBA et caractérisées par la production d’acides gras à chaîne courte et la résistance aux antibiotiques. Un modèle cellulaire d’inflammation de type T2 utilisant nos cellules épithéliales d’enfant AS ou NA en présence de l’IL-4 a été mis en place, permettant d’analyser différentes fonctions de défense de l’épithélium en réponse à cette inflammation, comme le transport ionique et l’expression des mucines et des alarmines. De plus, dans l’optique de proposer de nouvelles approches thérapeutiques, nous avons étudié l’impact, dans ce modèle d’inflammation, d’une bactérie lactique déjà décrite comme ayant un potentiel immunomodulateur dans les maladies pulmonaires. Les résultats préliminaires indiquent que l’IL-4 diminue l’activité du canal épithélial sodique et augmente l’activité du canal chlorure CFTR. De manière intéressante, la bactérie semble rétablir l’activité contrôle de ce canal.Mes travaux contribuent à une meilleure caractérisation locale de l’asthme sévère chez l’enfant. En effet, notre analyse multi-omique a mis en évidence une relation étroite entre les signatures métabolomique et immunitaire, tout en identifiant des métabolites et des genres bactériens différemment régulés dans les poumons des enfants souffrant d’AS. Ces résultats originaux seront confirmés sur la cohorte SevAsthma, plus importante, actuellement en cours d’inclusion. Notre modèle épithélial in vitro d’inflammation de type T2 a aussi établi les bases de tests fonctionnels qui permettront l’évaluation de bactéries à potentiel probiotique dans la pathologie de l’asthme
