PSA study during androgenisation therapy in transgender men

Los hombres transgénero (TXM) son personas a las que se les asignó género ‘femenino’ al nacer y manifiestan una identidad de género masculina. Existe una carencia de datos de calidad en cuanto a la investigación de las consecuencias del uso a largo plazo de testosterona (terapia de afirmación de género), ya que los protocolos actuales promueven un tratamiento con andrógenos ilimitado. El objetivo principal es encontrar nuevos marcadores que permitan evaluar de forma objetiva el proceso de androgenización del hombre transgénero, para así ofrecer un tratamiento personalizado y prevenir efectos indeseados. Uno de los marcadores en estudio es el PSA, que incrementa su producción tras la terapia de androgenización. En nuestro estudio presentamos datos de 19 TXM en seguimiento cada 3 meses durante 1 año de tratamiento hormonal cruzado. Entre los parámetros básicos analizados en su control evolutivo se observa incremento estadísticamente significativo en el hematocrito, hemoglobina, número de hematíes y creatinina ya desde los 6 meses post tratamiento. También se incrementan los niveles de ALT, GGT y fosfatasa alcalina, siendo significativos desde los 9 meses. De manera específica analizamos los niveles de andrógenos y el PSA sérico, mostrando significación a partir del tiempo 6 y 9, respectivamente. La mayor originalidad de nuestro estudio ha sido la comparación frente a sujetos CIS tanto mujeres como hombres.Transgender men (TXM) are people who were assigned a 'female' gender at birth and manifest a male gender identity. There is a lack of quality data regarding the investigation of the consequences of long term testosterone use (gender affirming therapy), as current protocols promote unlimited androgen therapy. The main objective is to find new markers that allow us to objectively evaluate the androgenization process of transgender men, in order to offer personalized treatment and prevent unwanted effects. One of the markers under study is PSA, which increases its production after androgenization therapy. In our study, we present data from 19 TXM followed-up every 3 months during 1 year of crossover hormonal treatment. Among the basic parameters analyzed in their evolutionary control, a statistically significant increase in hematocrit, hemoglobin, number of red blood cells and creatinine was observed already from 6 months post treatment. The levels of ALT, GGT and alkaline phosphatase also increase, being significant from 9 months. Specifically, we analyzed androgen levels and serum PSA, showing significance from time 6 and 9, respectively.
