Corrélats anatomiques et fonctionnels de la réponse clinique au traitement par Stimulation Magnétique Transcrânienne répétée (rTMS) chez des patients atteints de schizophrénie avec des hallucinations auditives résistantes

Auditory Verbal Hallucinations (AVHs) are underpinned by alterations in structural and functional brain connectivity in both the language network and the Default Mode Network (DMN). Repetitive Transcranial Magnetic Stimulation (rTMS) is a non-invasive technique with therapeutic potential for the treatment of symptoms resulting from disturbances in brain connectivity such as AVHs. However, its therapeutic benefits are moderate and highly variable from patient to patient. The aim of this thesis was to better characterize the clinical response to rTMS treatment by studying the structural and functional correlates within the language network and DMN in patients with schizophrenia.Our findings showed a lower structural connectivity in the left arcuate fasciculus in Responder patients to rTMS treatment compared to Non-Responder patient, which was correlated with a reduction in AVHs after rTMS. Treatment with rTMS at 1 Hz over the Wernicke area reduced the AVHs. We reported increased resting-state functional connectivity between the Wernicke area and the right precuneus in the Responder group and decreased functional connectivity between both regions in the Non-Responder group. The functional connectivity between these regions was correlated with the reduction of AVH severity. A modelling study of the effect of TMS did not lead to a better understanding of the clinical improvement of AVHs.The structural connectivity of the left arcuate fasciculus could be a predictive marker of clinical response to rTMS. Our work suggests research perspectives to develop personalized therapeutic protocols for each individual.Les hallucinations auditives verbales (HAV) sont sous-tendues par des altérations de la connectivité cérébrale structurelle et fonctionnelle dans le réseau du langage et dans le réseau par défaut (DMN). La Stimulation Magnétique Transcrânienne répétée (rTMS) est une technique non-invasive présentant un potentiel thérapeutique pour le traitement de symptômes issus de perturbations de la connectivité cérébrale comme les HAV. Toutefois, ses bénéfices thérapeutiques sont modérés et hautement variables d’un patient à l’autre. L’objectif de cette thèse était de mieux caractériser la réponse clinique au traitement par rTMS en étudiant les corrélats structurels et fonctionnels au sein du réseau du langage et du DMN chez des patients atteints de schizophrénie.Nos résultats ont montré une connectivité structurelle plus faible dans le faisceau arqué gauche chez les patients Répondeurs comparés aux Non-Répondeurs, corrélée avec l’amélioration des HAV après rTMS. La rTMS à 1 Hz sur l’aire de Wernicke réduisait la sévérité des HAV. Nous observons une augmentation de la connectivité fonctionnelle entre l’aire de Wernicke et le précunéus droit dans le groupe Répondeur et une diminution de connectivité entre ces deux régions dans le groupe Non-Répondeur. La connectivité entre ces deux régions était corrélée avec l’amélioration des HAV. Une analyse de modélisation de l’effet de la TMS n’a pas permis de mieux comprendre l’amélioration des HAV.La connectivité structurelle du faisceau arqué gauche pourrait être un marqueur prédictif de la réponse clinique à la rTMS. Nos travaux suggèrent des pistes de recherche pour développer des protocoles thérapeutiques personnalisés pour chaque individu
