Medication administration errors are common, costly and the cause of adverse events in clinical practice. Interruptions during medication administration rounds are thought to be a prominent causative factor of these errors. The change chosen for this project was the introduction of drug round tabards in a long term care facility for the elderly. The aim was to reduce non-urgent interruptions during drug rounds, reduce the incidents of medication errors, enhance patient safety, safe time and promote compliance with professional and national standards on medication management.
