Helicobacter pylori: patología y prevalencia en biopsias gástricas en Colombia

Comparative study describing the prevalence of H. pylori and associated alterations of gastric mucosa in symptomatic individuals from the main capitals of Colombia. Aim. To estimate the prevalence and describe the pathology associated to the infection by H. pylori in endoscopic biopsies of gastric mucosa. Methodology. The results of all histopathological reports from 1997 were revised in the departments of pathology located in the capitals of 16 different departments around the country. The requested studies were selected to evaluate gastric pathology in order to collect the available information about age, gender, type of procedure (biopsy or gastrectomy) and histopathological diagnosis. A description of these characteristics was made in the different regions of Colombia. Results. A total of 86,243 histopathological studies from 1997 were realized in the Regional hospitals of the 16 participant cities. The 10.4% belonged to stomach samples obtained in the 96.5 % of cases by endoscopic biopsy and the 3.5% remaining by gastrectomy. The prevalence of H. pylori was 69.1%. The most frequent histopathological diagnosis was chronic atrophic gastritis in 36.4%. Only in 1.4% of cases, gastric mucosa was normal. The prevalence of cancer was 9.3% and that of gastric ulcer 5.1%. The 96.9% of malign tumors were carcinomas and only 3.1% lymphomas. The cities located in the mountainous regions of Colombia (Andean region) had significantly higher prevalence of multifocal atrophic gastritis compared to the Atlantic cost (34.1% vs. 6.4%) (p < 0.001). In a similar way, in the Andean region the prevalence of gastric cancer was higher than that of the Atlantic cost (9.7% vs. 6.7% p < 0.001). Differences in frequency of ulcer and displasia were not observed. Conclusion.In Colombia, significant geographic variations are observed in the distribution of gastric carcinoma and the complex of precursor lesions: multifocal atrophic gastritis, intestinal metaplasia and displasia. RESUMEN Estudio comparativo que describe la prevalencia de Helicobacter pylori y las alteraciones asociadas de mucosa gástrica en individuos sintomáticos procedentes de las principales capitales de Colombia. Objetivo.Estimar la prevalencia y describir la patología asociada con la infección por Helicobacter pylori en biopsias endoscópicas de mucosa gástrica. Metodología.En los departamentos de patología de los hospitales regionales localizados en las capitales de 16 departamentos de Colombia se revisaron los resultados de todos los informes histopatológicos practicados durante 1997. Se seleccionaron los estudios solicitados para evaluar las enfermedades gástricas para recolectar la información disponible sobre edad, género, tipo de procedimiento (biopsia o gastrectomía) y diagnóstico histopatológico. Se hizo una descripción de estas características en las diferentes regiones de Colombia. Resultados.En los hospitales regionales de las 16 ciudades participantes se realizaron durante 1997 un total de 86.243 estudios histopatológicos; 10.4% correspondieron a muestras de estómago obtenidas en 96.5% de los casos mediante biopsia endoscópica y en el restante 3.5% mediante gastrectomía. La prevalencia de H. pylori fue 69.1%. La mucosa gástrica fue normal en 1.4% de los casos. El diagnóstico histopatológico más frecuente fue gastritis crónica no atrófica en 36.4%. La prevalencia de cáncer fue 9.3% y de úlcera gástrica 5.1%; 96.9% de los tumores malignos fueron carcinomas y sólo 3.1% linfomas. Conclusión. La patología gástrica asociada con la infección por H. pylori es una causa frecuente de morbilidad en Colombia y su distribución muestra variaciones geográficas significativas
