¿La episiotomía debe ser un procedimiento de rutina?

Introduction: The episiotomia as an obstetric procedure had it first roots at the start of the 18th century, when Stephen mentions how Old made for the first time the episiotomia for extremely difficult delivery. Goals: Comparing the presence of some obstetric complications in nuliparas women after practicing either or not the episiotomia mediolateral. Methodology: The work was donde in the Carlos Holmes Trujillo hospital, that covers the attention of low risk deliveries it south eastern part of the city. An experimental design was made in the one that were selected two different groups of gestantes. The muestra was conformed by 34 patients for the group control (who received the rutinary management of making episiotomia) and 30 patients for the experimental group (the ones that didn’t received the routine episiotomia). Results and discussion: The harm perception was a much more intense in thr control group with significatively different status; the pain was qualify form modered to severe. In none of the groups were presented desgarros (grade III and IV) circumstances that can be related with the homogeneity that presented the groups according to the different variables associated with this factor, no matter what were some of them presented desgarros in grade I and II in the experimental group occurred in its mayority in the deliveries attended by the less experimented people. The harm associated with episiotomy valued in ten days post delivery were more in the control group against the experimental group, especially difficult to sit downand fear the restart sexual activity, finding frequencies until four timesigher in the control group, circumstances that can affect the process of cicatrisation in the partner relation. This findings are related with the surgical incision and the disagreement things produced in the puerperium. When evaluating the presence of dispareunia in both groups at the sixty days postpartum it was observed: In the control group some soft harms as pain and bleeding with the start of sexual activity (15 post delivery). The strength of the present study is based precisely in relatives aspects to the selective episiotomy. Conclusions: It exists a group of patients that were beneficiated in an important way from the episiotomy, reason why it could not be abolished completely in this procedure; but the literature in the present study coincides in suggest that used in a selected way with much more cleared and specifics concepts (imminence of laceration or compromise of fetal vitality) produce more benefits to the women. RESUMEN Introducción: La episiotomía como procedimiento obstétrico tuvo sus inicios en el siglo XVIII y desde 1920 hasta hoy es usada como procedimiento de rutina en primigestantes. Objetivo: Identificar algunas complicaciones obstétricas en mujeres nulíparas después de practicar o no la episiotomía mediolateral. Metodología: El trabajo se realizó en el Hospital Carlos Holmes Trujillo en el suroriente de Cali. Se utilizó un diseño experimental con selección aleatoria de dos grupos de gestantes. La muestra estuvo conformada por 34 pacientes para el grupo control (quienes recibieron el manejo rutinario de realizar episiotomía) y 30 pacientes para el grupo experimental (a quienes no se les realizó episiotomía de rutina). Resultados y discusión: La percepción dolorosa fue más intensa en el grupo control con diferencias estadísticamente significativas; el nivel del dolor se calificó con la escala visual análoga . En ninguno de los grupos se presentaron desgarros severos; sin embargo, se presentaron desgarros grado I y II en el grupo experimental ocurridos en su mayoría en los partos atendidos por el personal menos experimentado. Las molestias asociadas con la episiotomía valorada a los 10 días postparto fueron hasta cuatro veces mayores mayores en el grupo control sobre el grupo experimental, sobre todo dificultad para sentarse y temor para reiniciar la actividad sexual, circunstancias que pueden afectar el proceso de cicatrización y la relación de pareja. Al evaluar la presencia de dispareunia en ambos grupos a los 60 días postparto se observó en el grupo control molestias como ardor, dolor y sangrado con el inicio de la actividad sexual; en el grupo experimental el inicio de la actividad sexual fue más precoz (15 días postparto). La fortaleza del presente estudio radica precisamente en estos aspectos relativos a la experiencia de la mujer a quien se le realizó episiorrafia; es de vital importancia conocer la forma de realización del procedimiento y cómo afecta su vida después del egreso hospitalario. Conclusiones: Existe un grupo de pacientes que se benefician en forma importante de la episiotomía, motivo por el cual no se puede abolir completamente este procedimiento; pero la literatura y el presente estudio coinciden en sugerir que cuando se usa de manera selectiva con inminencia de desgarro o compromiso de la vitalidad fetal produce mayores beneficios a la gestante
