A pilot field study to test the applicability of physiological biomarkers for phycological stress and health effects. The variation of and the influence on cortisol, serotonin, and neopterine in urine of volunteers

This report gives a brief overview of the literature on the relationship between noise exposure and health effects. The study was based on the hypothesis that exposure to noise is associated with psychological stress and that such effects can be identified by physiological health effects indicators. The aim of the pilot study was to introduce and test the applicablility of indicators. The aim of the pilot study was to introduce and test the applicability of inidcators for personal exposure and if possible human healt effects as a supplement to the more broadly used quextionnaires for annoyance and sleep disturbances and to identify determinants that irrespective of the exposures of interest, i.e. noise may affect eht health indicators. This was tested in a pilot field study, in which a group of 10 volunteers collected moring urine during a period of 14 consecutive days and also filled out questionnaires. Based on the literature survey, a choice was made to measure three stress-related hormones in urine: cortisol, neopterine and serotonine. The concentrations of the hormones were determined. Based on these data, mean and variation of the parameters were calculated. This analysis revealted that from the three hormones measured in this study, cortisol seems to have the largest relative variation. Also, the number of days that urine will be collected seems not to affect the signal to noise ratio of serotonine. A simplified model was applied to estimate the size of a panel and the duration of the sampling period to detect a significant effect of a stressor (presuming that the range of an effect is known). A linear regression model was applied to edentifiy the effects of potential confounders. Smoking and alcohol is associated with cortisol, serotonine and neopterine urine levels. The moment that a person falls asleep is associated with cortisol levels. The use of palliatives is positively associated with neopterine levels. After controlling for potential confounders, a positively rated sleep quality was statistically significantly associated with the cortisol and almost with serotonine levels. No assosiations were detected for neopterine. In conclusion, this pilot field study shows that cortisol, neopterine en serotonine can be as measured in urine. However, the subjects in this study also experienced the type of sampling also as a burden and intrusive in day-to-day life. Smoking, alcohol consumption, gender and the use of medicines are confounders. Sleep quality was statistically significantly associated with cortisol concentrations in urine. It has yet to be established in what proportion a stressor (i.e. noise) can affect these hormones (exposure-effect relationships) and what the meaning of such an effect is for the health status of the human population.Dit rapport geefte een beknopt overzicht van de literatuur over de relatie tussen blootstelling aan geluid (lawaai) en de gezondheidseffecten, gericht op stress en slaapverstoring. De studie was gebaseerd of de hypothese dat expositie aan lawaai invloed kan hebben op psychologische stress en dat dergelijke effecten geidentificeerd kunnen worden dmv fysiologische indicatoren. Het doel van deze voorstudie was het introduceren en het testen van de bruikbaarheid van deze blootstelling en zo mogelijk gezondheidsindicatoren als toevoeging voor het veel vaker gehanteerde vragenlijstonderzoek voor hinder en slaapverstoring. Dit is onderzocht in een veldstudie, waarbij een groep van 10 vrijwilligers gedurende een periode van 14 dagen urine heeft verzameld en vragenlijsten hebben ingevuld. Het moet echter nog vast worden gesteld in welke mate een stressor (bijv. lawaaio) invloed heeft op deze hormonen (blootstelling-effect relatie) en wat de betekenis is van een zodanig effect op de gezondheid
