A healthy judgement for the Netherlands? Analysis of international comparative reports on health care and health

In the year 2000 several reports appeared which analysed Dutch health care and health status from an international perpspective. These include the 'World Health Report 2000: Health Systems: Improving Performace', a country-report for the Netherlands by the OECD, and two 'national' reports, by the Social and Cultural Planning Bureau (SCP) and by the Ministry of Social Affairs and Labour (SZW). The Ministry of Health Welfare and Sports (VWS) has asked the RIVM to analyse these reports. Methods and data were analysed as well as the scope and concepts that were used and the results were compared with a view on their importance for Dutch health policy. WHO has developed a new and visionary model to describe 'health system performance' of all 191 member countries. The broadly defined concept of a 'health system' includes all actions contributing to better health. Five indicators were used: Health status (level and distribution), Responsiveness of the system (level and distribution) and 'fairness' (of financial contribution). The methods are the subject of much discussion and the exercise has been characterised by a substantial lack of real data, which has been substituted by estimates and extrapolations. On this basis, the Netherlands ended in a 17th position in a world ranking list. No clear messages for national health policy can be derived from this result, however. Still, it is recommended to actively participate in further development of the concept and to improve the delivery of national data. Parallel developments are going on in a more transparent fashion within the OECD framework. Some of the conclusions from these reports, namely that Dutch life expectancy and mortality around birth are improving more slowly than in other countries of the European Union, has urged the Ministry of Health request the RIVM for two more detailed analyses of these unfavourable trends.In 2000 verschenen een aantal rapporten die de Nederlandse zorg en gezondheid vanuit internationaal perspectief beschreven. Dit waren: het 'World Health Report 2000: Health Systems: Improving Performance', een landen-rapport van de OECD and twee Nederlandse rapporten van het Sociaal en Cultureel Planbureau (SCP) en het ministerie van SZW. Het ministerie van VWS heeft het RIVM gevraagd om deze rapporten te analyseren. De gebruikte methoden en gegevens zijn vergeleken alsmede de reikwijdtes en gebruikte concepten en de resultaten werden vergeleken met het oog op hun bruikbaarheid voor het Nederlands beleid. De WHO heeft een nieuw en visionair model gepresenteerd om de prestaties van de gezondheidssystemen van alle 191 lidstaten te beschrijven. Het breed gedefinieerde concept 'health system' omvat alle activiteiten die aan een betere gezondheid bijdragen. Vijf indicatoren zijn gebruikt: Gezondheid (niveau en verdeling), 'Responsiveness'(client-gerichtheid) (niveau en verdeling) en 'fairness (eerlijkheid) of financial contribution'. De gebruikte methoden zijn onderwerp van veel internationale discussies en de onderneming wordt gekenmerkt door een substantieel gebrek aan echte data, die door schattingen en extrapolaties vervangen zijn. Op de basis hiervan eindigde Nederland op een 17e plaats in een wereldranglijst. Er kunnen echter geen duidelijke boodschappen voor het beleid aan dit resultaat ontleend worden. Toch wordt aanbevolen om actief te participeren in de verdere ontwikkeling van deze concepten en om de nationale dataleverantie te verbeteren. Parallel hieraan zijn er vergelijkbare ontwikkelingen bij de OECD, die op iets meer transparante wijze plaatsvinden. Enkele van de conclusies uit de hier boven aangeven rapporten, namelijk dat de Nederlandse levensverwachting en zuigelingensterfte zich relatief slecht ontwikkelen heeft het ministerie van VWS ertoe gebracht om het RIVM te vragen om hierover twee meer gedetailleerde analyses op te stellen
