The morbidity of asthma and Chronic Obstructive Pulmonary Disease in the Netherlands: additional analysis and update of available data

This study, commissioned by the Netherlands Asthma Fund, fills gaps in the knowledge on the morbidity of asthma and chronic obstructive pulmonary disease (COPD) identified in an earlier survey. Specific objectives were to: 1) integrate newly available data on trends in asthma and COPD from general practice registers and 2) to fill gaps in the information, both on the prevalence of combinations of asthma and COPD characteristics, and on socio-economic and ethnic differences in asthma and COPD, through secondary analysis of population-based studies. The update of the general practice registers showed that the prevalence of asthma diagnosis had increased from 5 per 1000 persons in 1983 to 26-31 per 1000 persons in 1999. The prevalence of a chronic bronchitis/COPD diagnosis had decreased slightly among men between 1975 and 1999, while women showed a strong increase of 10 per 1000 around 1980 to 19 per 1000 in 1999. Secondary analyses in population-based studies on airway symptoms in combination with clinical characteristics of asthma and COPD showed: - the prevalence of asthma symptoms in combination with airway hyper-reactivity and atopy in 8-12 year-old children to be about 3.5%; - the prevalence of COPD symptoms in combination with a decreased lung function to be 2% in adult men and 1% in adult women. The analysis of respiratory symptoms in combination with clinical characteristics of asthma and COPD provided a better picture of respiratory problems underlying the prevalence of asthma and COPD registered by general practitioners Secondary analysis of socio-economic and ethnic differences in asthma and COPD showed: - no socio-economic differences in the prevalence of asthma in children and adults; - the prevalence of COPD in highly educated adults to be lower than in lower educated adults. Socio-economic differences in smoking habits explained an important part of the socio-economic differences in COPD. - no ethnic differences in the incidence of doctor-diagnosed COPD. In conclusion, additional analysis with existing databases without new data collection fills several important gaps in knowledge on the morbidity of asthma and COPD in the Netherlands.Dit rapport geeft inzicht in prevalentie en incidentie van astma en COPD in aanvulling op een eerder inventariserend rapport (Smit en Beaumont, 2000). Specifieke doelen waren: 1) het integreren van recent beschikbare gegevens over trends in astma en COPD uit huisartsenregistraties 2) het vullen van leemtes in kennis over prevalentie van combinaties van kenmerken van astma en COPD en van sociaal-economische en etnische verschillen in astma en COPD, door secundaire analyse van populatiestudies. De actualisering van de huisartsenregistraties lieten zien dat de prevalentie van astma is toegenomen van 5 per 1000 personen in 1983 tot 26-31 per 1000 personen in 1999. De prevalentie van COPD was licht afgenomen bij mannen tussen 1975 en 19999, terwijl er bij vrouwen een sterke stijging werd waargenomen van 10 per 1000 rond 1980 tot 19 per 1000 in 19999. Secundaire analyse van epidemiologische populatiestudies liet zien dat: - De prevalentie van astma symptomen in combinatie met luchtweggevoeligheid en atopie in 8-12 jarige kinderen rond 3,5% was; - De prevalentie van COPD-symptomen in combinatie met een verlaagde longfunctie ongeveer 2% was in volwassen mannen en 1% in volwassen vrouwen. De analyses van respiratoire symptomen in combinatie met klinische kenmerken van astma en COPD gaf een beter beeld van de respiratoire problemen die ten grondslag liggen aan de prevalentiecijfers die in huisartsenpraktijken worden geregistreerd. Secundaire analyse van sociaal-economische en etnische verschillen in astma en COPD lieten de volgende resultaten zien: - er waren geen sociaal-economische verschillen in de prevalentie van astma in kinderen en volwassenen - De prevalentie van COPD in hoogopgeleide volwassenen was lager dan in laagopgeleide volwassenen. Verschillen in rookgewoonten en andere leefstijlfactoren verklaren een belangrijk deel van deze sociaal-economische verschillen; - Er waren geen etnische verschillen in de incidentie van de huisartsendiagnose COPD. De conclusie is dat de aanvullende analyses op basis van beschikbare gegevens zonder nieuwe gegevensverzameling, een belangrijke bijdrage heeft geleverd aan het vullen van leemtes in kennis over de morbiditeit van astma en COPD in Nederland
