Intercommunalité, mobilité et santé, l’intérêt du diagnostic partagé entre acteurs : le cas d’un établissement public de coopération intercommunale.

International audienceThere is a real challenge in terms of public health to address the action as close as possible to the citizens who benefit from it. In France, health policy has historically been characterised by centralisation and concentration. It is now moving towards a territorialisation of action, admittedly still timid but very tangible, described by some as a paradigm shift. This evolution is accompanied by an acculturation to new ways of producing local health initiatives. It is a fertile ground for developing the Health in All Policies (HiAP) approach, which is based on the capacity of stakeholders to collectively identify specific territorial problematic situations, which can be addressed through an intersectoral approach and the de-steering of local public policies. Based on the case study of a French public establishment of inter-municipal cooperation, we show how a shared local health diagnosis, built along the way and using cartographic representations, allows for the consensual identification of a problematic public health situation around the lack of physical activity and sedentariness. The shared local diagnosis is revealed to be an integrating tool for local public policies, allowing the setting in motion of a local intersectoral collective action dynamic.Il y a un véritable enjeu en termes de santé publique à s’emparer de l’action au pus proches des citoyens bénéficiaires. En France, la politique sanitaire est historiquement marquée par une centralisation et une concentration importante. Elle évolue vers une territorialisation de l’action, certes encore timide mais bien palpable, que certains qualifient de changement de paradigme. Cette évolution va de pair avec une acculturation à de nouvelles façons de fabriquer l’action sanitaire locale. Elle constitue un terrain propice au développement de l’approche Health in All Policies (HiAP), laquelle repose sur la capacité des acteurs à identifier collectivement des situations territoriales problématiques dont le traitement peut donner lieu à une approche intersectorielle et un dé-silotage des politiques publiques locales. A partir de l’étude du cas d’un établissement public de coopération intercommunale français nous montrons comment un diagnostic sanitaire local partagé construit chemin faisant et empruntant des représentations cartographiques permet de dégager consensuellement une situation de santé publique problématique autour du manque d’activité physique et la sédentarité. Le diagnostic local partagé se révèle être un outil intégrateur des politiques publiques locales permettant d’enclencher une dynamique d’action collective locale intersectorielle
