Can we prevent the Medication Errors? Implement Safety Measures

‘Medication error’ is any preventable event that may cause inappropriate use of medication and may cause to the patient. There are numerous medication error happened in hospital set up and need of analysis and tight control on it is necessity of organization to prevent patient harm. In this study, total medication error from April to July 2018 was collected and analyzed for the causes like wrong dose prescribed in drug chart, specific instruction not mentioned such as dilution or route of drug administration and complete order was not transcribe in to the new drug chart. Such errors owing to patient harm or else it was near miss error. After analyzing the problems, root cause done based on type of error and new strategy developed. This strategy discussed in pharmacy drug and therapeutic committee and approved by all members of committee. The new method include RTPA,( Recombinant Tissue Plasminogen Activator) error specific training, MMU (memory management unit) awareness, medication tracer and on floor training for doctors as well as nurses and pharmacist. In second phase of study, August to November 2018 clinical audit done as per MMU NABH and JCI. All pharmacy auditors were strictly stuck to new strategies during ward audit. After four months of hard implementations of newly developed strategy result calculated, total medication errors related to prescription were drastically decreased, in addition to this transcription errors also drops in count. This new process found to be effective and need to continue throughout the hospital
