In this article, we describe an evidence-based stepped care approach to improving the care of chronic illness in organized health care systems. A primary care physician’s goal is to deliver the care that’s right for patient not employ a one-size-fits-all approach. Tailored health care is easier when patient has meaningful relationship with his/her healthcare physician. A primary health care physician is responsible for screening all major health-related conditions. If patient already has a chronic condition, patient’s primary helps manage it and improve the quality of life.
