Kharkov National Medical University, Kharkov, Ukraine Summary Organ Preservation Surgery (OPS) is a viable and safe treatment for large kidney tumors. For large tumors (T2 or more), with OX, renal function is better preserved and therefore kidney resection has advantages over nephrectomy. Given that loss of kidney function can cause an increased risk of severe cardiovascular disease and mortality, OPS can have significant advantages over nephrectomy in terms of overall survival. OPS has a higher potential risk of peri-and postoperative complications than with nephrectomy due to the greater complexity of the operation, which requires a balanced approach in assessing the possible risks and advantages of the operation. However, the high risk of complications justifies itself, given similar oncological results and better preservation of renal function. OPS in patients with stage T2 and more should be used in well-selected patients, taking into account the characteristics of the tumor and the technical capabilities of the surgeon to reduce the number of complications. Based on this, further promising randomized trials are needed to evaluate the role of OPS in the treatment of large tumors. Key words: kidney tumors; organ-sparing surgery; radical nephrectomy
