The importance of rational drug therapy is increasing with the aging of the population. Since one of the main reasons for inappropriate drug prescribing in older patients are also age-blind approaches that may be caused or may result in ageist practices, this article describes main results of the narrative literature review summarizing the barriers related to insufficient individualization of drug regimens, age-blind approaches and ageist practices in medication use and medication prescribing in older patients
