The Agreement of SphygmoCor XCEL and VICORDER Measures of Central Hemodynamics

Cardiovascular disease (CVD) is the number one cause of death in the United States and among the leading causes of mortality globally. Several studies have suggested that CVD risk can be predicted by an increase in aortic wave reflection. Thus, assessment of central hemodynamics could be an important clinical tool for not only predicting cardiovascular risk but also determining vascular health. Pulse Wave Analysis (PWA) is a common non-invasive measurement technique that can be used to evaluate aortic wave reflections. The SphygmoCor XCEL and VICORDER are two widely used devices that have a PWA function, but whether both devices generate comparable PWA outcomes is still unknown. The SphygmoCor XCEL is the current gold-standard device for PWA, but it is also expensive and requires extensive operator training, while the VICORDER is more cost-effective and easier to use. The agreement between the two devices would provide clinicians and researchers with multiple options to assess aortic wave reflection using PWA. In addition, since both devices are widely used, future researchers will be able to harmonize PWA data between the two devices across multiple studies. Therefore, the objective of this study is to compare and determine the agreement of PWA outcomes between the SphygmoCor XCEL and the VICORDER devices. Based on our results, we found that there is no meaningful agreement between the devices for PWA. The overall intraclass correlation (ICC) between devices for PWA outcomes were significantly below 0.9, which is our aim for a meaningful agreement. We suggest that future researchers and clinicians utilize the SphygmoCor XCEL as it is the current gold standard and most widely used device across multiple studies, rather than the VICORDER to assess aortic wave reflection indices and predict cardiovascular disease and health.Bachelor of Art
