The surgical treatment of patients with Spinal hydatid cysts in the thoracic and lumbar segments

The Purpose is to analyze the patients who has tumors of the spine in the thoracic and lumbar spine and present the results of surgical treatment. The results of the research and treatment of 51 patients with tumors of the spine, conducted between 2009 and 2011. We studied the intensity of pain (VAS), a neurological condition (scale ASIA), and radiographic parameters which characterizing the degree of deformation of the spinal column. The Results. Excellent result of treatment (group A) was observed 14 (27%) patients. Good (group B) and satisfactory (Group C) results of treatment are marked 9 (19%) and 14 (27%) patients respectively. In 14 (27%) patients received poor treatment outcomes (group D). In this group died 2 (4%) patients, the Analysis of neurological disorders in groups A and В on the lower extremities showed improvement (on average) to 1,04 ± 0,56 degree scale ASIA / IMSOP. Regression of neurological disorders occurred all 11 patients with primary tumors of the spine. Marked decrease in pain (on average) to 3,17 ± 1,07 degrees in VAS. In groups A and С decrease in the angle of local kyphosis was (on average) 10,91° + 6,44°. The best results were observed in vertebral compression index (9,45% ± 8,50%), than in vertebral strain wedge index (6,55% + 9,86%).Conclusion. The Operation of posterolateral approach with the stabilization of the spine improved neurological status and / or local kyphosis angle for tumors to 73% of cases. In remote control studies have documented good results of treatment in 27% of patients with tumors of the spine.Цель работы - провести анализ больных с эхинококкозом позвоночника в грудном и поясничном отделах и изучить результаты хирургического лечения. Материалы. Представлены результаты исследований и лечения 26 больных с эхинококкозом позвоночника, проведенных в период с 2010 по 2014гг. Изучалась интенсивность болевого синдрома (шкала ВАШ), неврологическое состояние (шкала ASIA), рентгенологические показатели, характеризующие степень деформации позвоночного столба. Результаты. Хороший результат лечения(группа А) отмечен у 17 (65,4%) больных. Удовлетворительный (группа В) результат лечения отмечен у 8 (30,8%) больных. В 1 (3,8%) случае получен плохой результат лечения (группа С). Анализ неврологических нарушений в группах А и В по функции нижних конечностей выявил улучшение (в среднем) на 1,46+0,77 степени по шкале ASIA/IMSOP. Регресс неврологических нарушений имел место у всех 25 больных. Уменьшение болевого синдрома отмечено (в среднем) на 4,35+1,33 степени по шкале ВАШ. В группах А и С уменьшение угла локального кифоза составило (в среднем) 11,87°±7,12°. Лучшие результаты отмечались у ПКП (9,45%+8,50%), чем у ПКДП (7,55% +8,86%). Выводы. Операция заднебоковым доступом с удалением эхинококковых кист позвоночного канала, резекция пораженных эхинококкозом тел позвонков с надежной внутренней стабилизацией уменьшает болевой синдром и уменьшает неврологические нарушения. В отдаленных контрольных исследованиях зарегистрирован хороший результат лечения у 81,1% больных с эхинококкозом позвоночника
