Introduction: Down syndrome is the most common human chromosomopathy and the main cause of congenital intellectual disability, although it can occur at any maternal age, age over 35 years has been established as one of the main risk factors, currently it has been Established screening programs to improve early prenatal detection rates, the Expanded Combined Test screening, which consists of primary and secondary ultrasound markers and biochemical markers associated with maternal age, has a detection rate of 93% with a false positive rate of 2.5%. Objective: To describe the case of a patient with early detection of high risk of Down Syndrome in the pregnancy through the extended combined test of the first trimester and its confirmation in the newborn. Materials and methods: Descriptive, retrospective study, clinical case report. Results: A 43-year-old pregnant patient with no significant pathological history. In the first trimester screening, a high risk of Down syndrome of 1/4 was reported with the use of the expanded combined test, although the confirmatory cytogenetic study test was not performed, the patient attended the delivery with a high suspicion of a fetus with trisomy 21. At birth, the diagnosis is verified with the neonate's phenotype and confirmed with the 47, XX+21 karyotype study. Conclusions: Although chromosomopathies increase with maternal age, it is recommended to carefully screen all pregnant women in the first trimester for aneuploidies, applying the extended combined test due to its high detection rate and low false positive rate.Introducción: El síndrome de Down es la cromosomopatía humana más común y la principal causa de discapacidad intelectual congénita, aunque puede presentarse a cualquier edad materna, se ha establecido la edad sobre los 35 años como uno de los principales factores de riesgo, actualmente se ha establecido programas de cribado para mejorar las tasas de detección prenatal temprana, el screening de Test combinado ampliado, que consta de marcadores ecográficos primarios y secundarios y marcadores bioquímicos asociados a la edad materna presenta una tasa de detección del 93% con una tasa de falsos positivos del 2.5%. Objetivo: Describir el caso de una paciente con detección temprana de alto riesgo de Síndrome de Down en el embrazo mediante el test combinado ampliado del primer trimestre y su confirmación en el recién nacido Materiales y métodos: Estudio descriptivo, retrospectivo. Reporte de caso clínico. Resultados: Paciente gestante de 43 años, sin antecedentes patológicos de importancia. En screening del primer trimestre se reporta alto riesgo de Síndrome de Down de 1/4 con el uso del Test combinado ampliado, aunque no se realizó la prueba confirmatoria de estudio citogénetico la paciente acude al parto con alta sospecha de feto con trisomía 21. Desde el nacimiento se verifica el diagnóstico con el fenotipo del neonato y se confirma con el estudio de cariotipo 47, XX+21. Conclusiones: Aunque las cromosomopatías aumentan con la edad materna se recomienda realizar el screening de aneuploidías cuidadosamente a todas las gestantes en el primer trimestre, aplicando el test combinado ampliado por su alta tasa de detección y baja tasa de falsos positivos
