Osteomielitis hematógena aguda en pediatría

Acute hematogenous osteomyelitis is a common infection in pediatrics. Most cases occur in healthy children; however, there are factors that increase the risk of suffering from this disease. Even though its pathogenesis is multifactorial and not completely understood; the most common etiologic agent is Staphylococcus aureus. However, there are others that should be considered depending on the patient’s age and risk factors. The clinical presentation is usually variable, so it is essential to have high clinical suspicion for the diagnosis. Clinical manifestations, inflammatory markers, imaging studies, and cultures are considered for the diagnosis. The treatment with antibiotics must be given for at least 4 weeks; it is started empirically, targeting the most frequent pathogen and according to the susceptibility for the geographical site. Usually, patients have a very good prognosis and complications are reported in less than 10% of cases; considering that the biggest risk factor is inadequate treatment of the acute infection. This article consists of a bibliographic review on the generalities of acute hematogenous osteomyelitis in pediatric patients, with a focus on etiology, microbiology, and management.La osteomielitis hematógena aguda es una infección frecuente en pediatría. La mayoría de los casos se presentan en niños sanos; sin embargo, existen factores que aumentan el riesgo de padecer esta enfermedad. A pesar de que su patogénesis es multifactorial y poco comprendida, el agente etiológico más frecuente es el Staphylococcus aureus. Sin embargo, existen otros microorganismos que se deben considerar según la edad y los factores de riesgo del paciente. La presentación clínica suele ser variable, por lo que es imprescindible tener una alta sospecha clínica para el diagnóstico; se tienen en cuenta las manifestaciones clínicas, los marcadores inflamatorios, los estudios de imágenes y los cultivos. El tratamiento antibiótico tiene una duración mínima de 4 semanas y se inicia de manera empírica, dirigido al patógeno más frecuente y según la susceptibilidad en el sitio geográfico. El pronóstico usualmente es muy bueno y se reportan complicaciones en menos del 10% de los casos, siendo el tratamiento no adecuado el mayor factor de riesgo. Este artículo consiste en una revisión bibliográfica sobre las generalidades de la osteomielitis hematógena aguda en pediatría, con un enfoque en la etiología, microbiología y manejo
