Influence du climat institutionnel entourant la naissance sur les comportements et les interactions précoces entre la mère et son bébé

The present study is aiming at comparing the behaviors and interactions mother/ new-born, observed on the 4th or 5th day after birth, while the mother is breast-feeding her baby. The observations were carried on in three different obstetrical departments. One of these uses methods concerning birth which are said to be « traditional » (delivery with predominant medical concern, breast-feeding at preset hours, baby care preferentially taken over by nursing staff...). The other two obstetrical départements have introduced conditions favoring better surroundings during pre-, neo- and post-natal periods (prenatal preparation, both physical and psychological, baby laid on the mother's belly right after delivery, mother and father encouraged to take over baby care personnaly as soon and often as possible...). The results of the study demonstrate on the one hand that in the « traditionel » obstetrical department, the mothers are more centered on the technical aspect of feeding and the babies appear more « asleep », and therefore, the two partners show less interactional exchanges. In the other two obstetrical departments on the other hand, a richer and more varied communication is observed between mother and infant, communication which is modulated according to the personal features of the two partners and also to the institutional criteria caracterizing each on of these two obstetrical départements.Le présent travail vise à comparer les comportements et interactions mère-bébé observés le 4e ou 5e jour après la naissance, lors de la situation d'allaitement au sein, dans trois maternités différentes. L 'une d'elles utilise des méthodes entourant la naissance dites traditionnelles (accouchement où l'aspect médical domine, allaitement à « heure fixe », prise en charge préférentielle du bébé par le personnel soignant). Les deux autres maternités pratiquent des conditions susceptibles d'améliorer le climat des périodes pré-, néo- et post-natales (préparation prénatale tant physique que psychologique, bébé déposé sur le ventre de la maman dès après l'accouchement, mise au sein précoce, allaitement à la demande, place privilégiée de la mère et du père dans les soins au bébé). Les résultats obtenus montrent qu 'à la maternité traditionnelle, les mères plus centrées sur l'aspect technique de l'alimentation et les bébés « plus endormis » n 'ont que peu d'échanges interactionnels. Dans les deux autres maternités, en revanche, il existe une communication plus riche et variée entre les deux partenaires qui se module selon leurs caractéristiques personnelles et les critères institutionnels propres à chaque maternité.Gillot-De-Vries Francine, Wellemans-Camus Maggy, Candeur-Meyrant Simone.
